30 June 2009 PERSONAL INCOME TAX RETURN CHECKLIST We have provided you with this checklist to assist in gathering all your information for us to complete the 30 June 2009 income tax return. Please provide all required documentation. Please return the completed checklist with the required information. Information Required INCOME PAYG Payment Summary from Employers: Include ETP’s, Payment Summaries from Centrelink and Superannuation Pensions Interest Received: Provide details of the financial institution including, the account name, number and total TFN deducted. Dividends received: Copies of dividend advices – include dividends re-invested as DRPs Trust & Partnership Distributions: Provide copies of trust and partnership annual taxation summary statements Capital Gains/Losses on Property/Shares/Other Investments Contract and settlement statements for all purchases & sales (include all associated costs, eg., legal costs, agents commissions) Foreign Income: Details of overseas transactions (eg wages, interest, pensions, investments – include any tax paid) Other Income: eg., Primary Production (including stock on hand as at 30 June), business income RENTAL PROPERTY: For each rental property, please provide: Properties purchased/sold during the year: - Copy of purchase contract - Copy of settlement statement, including depreciation schedule & details of construction costs for depreciation purposes. - Copy of Bank Finance Confirmation - Stamp Duty - Legal Fees - Agents Commission - Borrowing Costs/Discharge Fees Current Year Information: - Property address : / / - Date first available for rent during the 2009 year (1/7/08–30/6/09) / / - Date last available for rent Gross Rents received & Other reimbursements Bank Loan Statements (include every page for the year) Agent Summary Rates & Land Tax Other Expenses such as Advertising, Insurance, Maintenance Levies, Pest Control, Inspection Fees and Stationery, Body Corporate - Repairs & Maintenance (Please provide details of all costs of repairs) km - Travel Expenses for inspections: Km’s Travelled cc Engine car size - Furniture & Fixtures Purchased or Sold: (Please provide details of expenditure including date of purchase & cost/proceeds of asset) If you require a detailed summary of depreciation entitlements, please contact our office. Other Deductions: eg Sickness & Accident Insurance TAX OFFSETS/REBATES Family Tax Benefit (FTB): From the 2009 financial year Family Tax Benefits will no longer be claimable through the tax system. All FTB claims must be lodged through the Family Assistance Office. Education Tax Rebate (ETR): You need to qualify for FTB Part A in order to claim the ETR. If you are eligible for the ETR please provide copies of all receipts for purchases made for eligible education expense (e.g. computers, computer accessories and software, textbooks, stationery, etc.) Baby Bonus: Did you have a baby on or after 1 July 2001 and before 1 July 2004? If so, please provide the child’s name and date of birth. Medical Expenses: Net medical expenses (including pharmaceuticals) over $1,500 (after refunds from Medicare & Health Funds) for yourself, spouse, children under 21, and dependant children under 25. Spouse Superannuation: Contributions on behalf of non-working or low income spouse.
