Guidance for filling out the Notice of Complaint form
We need specific information to ensure the quickest resolution. When making a complaint you
1. Your details: We need the following information to help us deal with your complaint faster:
− Correct contact details
− The email you check most
− Telephone (cell phone and home number)
− Student ID Number
2. Provide the name of the person/s you have already spoken to in relation to this issue.
3. Who is your complaint against? We need to know the name of the person or service so we
can investigate it.
4. Provide as many details as possible. While you may feel upset please keep to the facts and
We understand you may feel frustrated and upset. Complaints with a lot of emotion make it
difficult for us to follow the facts. We take all complaints seriously and will investigate fully.
Where necessary we will take steps to prevent problems occurring in the future.
