This is a scanned Allegheny County Court of Common Pleas notice to the tenant that he has not been paying enough rent into the court and that you will be seeking to terminate the supersedeas which prevents eviction. You have to mail this form at the post office and request a certificate of mailing (Form 3817). Also, there is a blank praecipe for certification of termination of supersedeas, which you would file after the 10 day notice.
