Student/Family Enrollment Information CHILD CUSTODY FORM 2008-2009 School Year Our ultimate goal is to provide a safe environment for all students. As a result, the School District of Milton implemented the "Student/Family Enrollment Information" form with regard to Child Custody matters as part of the enrollment process beginning with the 2004-2005 school year. In keeping with the stipulations set forth by this new procedure, a signed copy of the form, along with copies of any current custody order arrangements, are required to be kept on file for each child enrolled in the district. Returning students, for whom there is already a signed copy of the form on file, have two options as part of the 20082009 registration process. • No Changes/Modifications. On behalf of those returning students for which there are no changes in their child custody order arrangements since their last registration, no modifications in the paperwork provided at that time, and/or no need to provide any additional or updated paperwork, there is no need to respond at this time. • Changes/Modifications in Custody Orders. On behalf of those returning students for which there are changes in their child custody order arrangements since their last registration, copies of updated paperwork will need to be submitted during the registration process. Please complete the attached form and submit the signed copy, along with copies of current custody papers at registration. (Please note if you have only your original copy of the child custody order, you will have an opportunity to make a duplicate copy for the school during registration. Please also note that the school only requires copies of those pages of the decree pertaining directly to the child's custody arrangements, along with a signature page indicating the date of the order.) Note: A certified copy of any child custody order which has been issued with respect to the student, as well as any later court orders modifying the original custody order or arrangements, must be presented to the school district during registration. At that time district personnel will photocopy those pages of the decree pertaining to the custody order arrangements of the child, along with the signature/date page, and return the original copies of the document to you for your personal record keeping. Other parent is deceased. _____ Parents are divorced or legally separated. Student lives with the parent who was granted legal custody by court order. If this is your situation, the school must be provided a certified copy of the custody order or decree at the time of enrollment. _____ Parents are divorced or legally separated. Student lives with the parent who was not granted legal custody by court order. If this is your situation, you will be referred to the Superintendent before the student will be admitted to school. _____ Parents are divorced or legally separated and have joint or shared custody. If this is your situation, the school must be provided a certified copy of the custody order at the time of enrollment. _____ Parents are not legally divorced or legally separated. Student lives with only one parent, although no formal custody arrangements have been made at this time. _____ Student lives with neither natural parent, but lives with a guardian who has been granted custody by court order. If this is your situation, the school must be provided a copy of the court order at the time of admission. (This situation may also apply to a married student under age of 18.) _____ Student lives with guardian who has not been granted legal custody by court order. If this is your situation, you will be referred to the Superintendent before the student will be admitted to school. _____ Student lives with foster parents. If child was placed in your home by an accredited agency, a copy of the placement papers must be provided to the school at the time of admission. If child was not placed by an accredited agency, the school will refer you to the Superintendent before the student will be admitted to school. _____ Student is 18 years old or older and lives apart from his/her parents or guardian. _____ Student lives with single parent. Parent has never been married to the child's other biological parent and there has never been legal custody declared by court order. _____ Student lives with both biological parents. Legal custody has never been declared by a court order. _____ Tuition student. You must obtain a tuition agreement with current rates and payment schedule from the Central Office. _____ Foreign exchange student with J-1 visa. The district accepts foreign exchange students with J-1 visas who reside within the district, and are participants in an exchange program sponsored by accredited organizations screened and approved by administration. Exchange students on J-1 visas are not required to pay tuition. _____ Foreign exchange student with F-1 visa. Privately sponsored exchange students with F-1 visas may be enrolled if an adult resident of the district has been granted court appointed guardianship and the student lives in the home of that guardian. Exchange students with F-1 visas are required to pay tuition at the established district rate. Admission is limited to students at the secondary level who are sufficiently fluent in English and have met all immunization requirements. Once admitted, the length of enrollment may not exceed 12 months. _____ Other.
