I, the undersigned participant, hereby acknowledge the understanding that the
environment in which I will be in well enough to make sound judgment to protect my own
safety and that there are risks involved which are outside the control of T & J Canoe
Rentals. These risks involve loss of life, personal injury, and loss or damage to property. I
as a participant understand that problems may occur due to the decisions made by myself,
by bystanders or other users of the facilities or waterway where the activities are held. I,
the participant agree to wear a life vest at all times and acknowledge that I can swim. I
have been given a pre-launch briefing and given an opportunity to ask questions. I, the
participant fully understand and am fully aware that it is my option and responsibility to
terminate participation in this particular outing and will be given a complete refund.

I, the participant, agree to abide by all laws and waterway regulations while on said trip.
Glass containers are prohibited on the river. Littering is prohibited on the river. By law,
life vests are required for children thirteen years of age and under. By law, life vests are
required in the canoe per person for participants over the age of thirteen years old, (I have
agreed to wear my life vest at all times). I, the participant understand it is my responsibility
to be sure I have all required equipment prior to launching on said trip.

In consideration of T & J Canoe Rentals services to enable me to make said trips, I hereby
assume all risks of injury, all loss of life to myself and loss of or damage to property arising
out of my participation in such trips.

I further understand that T & J Canoe Rentals carries no insurance for the protection of
participants in these trips and any insurance coverage existing with respect to T & J Canoe
Rentals shall not alter the terms of this waiver nor impose any liability on T & J Canoe
Rentals and employees, each of whom are fully released from all liability by me.

 *** FINANCIAL LIABILITY*** I, the participant, understand that I am financially liable
for any loss or damage to rental equipment belonging to T & J Canoe Rentals due to
negligence on my part. I understand that a fee of $10 per hour will be charged after 5 PM.
