Credit Application: Personal Financial Statement

Description: This Personal Financial Statement should be used by lending institutions to evaluate an applicant's credit worthiness before a loan is made. The applicant (the person seeking credit) should fill out all required information, such as current assets, current liabilities, annual income, outstanding debt, credit card accounts, cash on hand, securities owned, life insurance, receivables due, and real estate owned. The form allows a co-applicant to provide information as well.
