2848 College Drive, Blair, NE 68008 June 2009 Dear Parents or Guardian, On behalf of the Dana College Athletic Training Department we would like to welcome you to Viking Athletics! We would like to take this opportunity to explain the medical coverage your student-athlete will be receiving while they participate at Dana College. You should keep this letter in a safe place for further reference. Dana College has entered into a service contract with Memorial Community Hospital and Health Systems in Blair, Nebraska to provide athletic training and physical therapy care for the student-athletes. For more information visit www.mchhs.org. Dr. David Buck from Heartland Orthopaedic and Sports Medicine of Fremont, Nebraska will continue to be the team physicians. Two certified athletic trainers employed by Memorial Community Hospital will provide the daily medical coverage for your student-athlete. A certified athletic trainer is a highly trained professional that specializes in the prevention, care, rehabilitation, and disposition of athletic related injuries. The certified athletic trainer works closely with the team physicians to ensure that the injured athlete is receiving the proper medical care. If your student-athlete requires medical attention beyond what the certified athletic trainer can provide, they will be referred to the appropriate medical care. Due to the federal regulations outline in The Health Insurance Portability and Accountability Act of 1996 (HIPPA) the task of informing you of an injury requiring further medical attention may be the responsibility of your student-athlete. Your student-athlete will sign a form that will give the athletic trainers specific instructions on who they can talk to about their injury. These include parents, coaches, faculty, school administrators, school insurance carrier, and the media. Although the legal age is 18, in Nebraska the age of majority is 19. Therefore, even though your student-athlete may be 19 or over, we ask that you sign the medical consent form allowing the athletic trainers to act in your behalf in case of a traumatic or life threatening injury. This consent will also allow the athletic trainer to act in your behalf for MRI’s, lab work and even surgery.

IMPORTANT INSURANCE INFORMATION PLEASE READ CAREFULLY AND KEEP FOR FURTHER REFERENCE It is the policy of the Dana College Athletic Department that all student athletes be covered by a major medical health insurance policy. No Student Athlete will be allowed to participate in any practice, conditioning, or competition until proof of insurance is confirmed. If you do not currently have an insurance policy, please contact Karyn Constant. Please notify us of any changes to a health insurance policy as soon as they occur – this includes but is not limited to changes in carriers or lapses in policy coverage. Dana College has acquired excess only athletic accident insurance to cover accidental athletic injuries directly related to participation in intercollegiate athletics during organized (meaning a staff coach, not student-coaches) practices or competition. Injuries sustained during physical education classes, intramurals, pick-up games or unsupervised workouts are not eligible for consideration under the athletic accident policy. Accidents outside of organized practices and competition fall under a different policy and have limited benefits. Please check with the Dana College Business office with any questions.

KEY POINTS ABOUT ATHLETIC ACCIDENT INSURANCE         INJURIES MUST BE REPORTED TO THE ATHLETIC TRAINING ROOM WITHIN 72 HOURS OF THE ACCIDENT. DANA COLLEGE, THE COACHES AND THE ATHLETIC TRAINERS DO NOT DETERMINE ELIGIBILITY OF CLAIMS. THE CLAIMS PROCESS IS THE RESPONSIBILITY OF THE STUDENT-ATHLETE AND THE PARENTS OR GUARDIAN. CLAIMS MUST BE FILED WITHIN 90 DAYS OF THE ACCIDENT. THE INJURY MUST BE DIRECTLY RELATED TO ORGANIZED PRACTICE OR COMPETION. PRE-EXIXTING CONDITIONS OR INJURIES MAY NOT BE ELIGIBLE FOR CONSIDERATION. THE STUDENT-ATHLETE MUST HAVE AUTHORIZATION PRIOR TO SEEKING MEDICAL ATTENTION. (YOU ARE NOT OBLIGATED TO USE HEARTLAND ORTHOPAEDIC.) YOUR STUDENT-ATHLETE MUST HAVE ACTIVE HEALTH/ACCIDENT INSURANCE IN FORCE DURING THE SCHOOL YEAR. FAILURE COULD RESULT IN ALL CLAIMS BEING THE RESPONSIBILITY OF THE STUDENT-ATHLETE AND PARENTS/GUARDIAN.

Dana College accepts your choice of health insurance carrier. Please make note on the proof of insurance form if your policy requires specific health care providers or sequences in obtaining health care. Failure to maintain adequate medical insurance will result in your student-athlete being declared medically ineligible to practice or compete. If you cannot afford nor have other circumstances regarding coverage please contact this office. If your student-athlete requires medical attention, you will need to submit the itemized statements and EOB’s (explanation of benefits) from your insurance company in order to file a claim. This office will assist you in filing the claim but does not take any responsibility for non-payment. You will be notified once about how to utilize the school’s policy. All incoming freshman and transfers will receive a pre-participation physical free of charge from the physicians and staff of Heartland Orthopaedic and Memorial Community Hospital at the designated time. Returning athletes will receive a mini-physical and extended follow-up if necessary. Check the physical schedule at the bottom of this page. These are mandatory; physicals from other physicians will not be accepted. If you have any questions regarding this letter please feel free to contact this office.
