The company trades from 1 Yew Tree Cottage, Hook Common, Hook, Hampshire RG27 9JJ. Studio XN Ltd is VAT Registered. Azur Make-Up will carry out the make-up/beauty/nail services in a professional and timely manner with the reasonable skill and care as such a service provider. As the client you will be kept informed of any changes in Azur Make-Up’s operation that may affect the make-up artist’s ability to perform the service. Each person receiving a make-up/beauty/nail service by Azur Make-Up/Azur Nail & Beauty will need to complete a consultation form. The form includes questioning on ALLERGIES, ADVERSE REACTIONS AND ILLNESS which could restrict the makeup/beauty/nail service being requested by Azur Make-Up. PRICES. Prices are subject to change without notice. Azur Make-Up reserves the right to amend prices given to cover any variation in cost which takes place after the date hereof. All price lists, quotes, order confirmation and invoices are in GB pounds Sterling. All prices include VAT at the current rate. OTHER COSTS. Parking charges & travels costs based on round trip from the salon studio in HOOK, RG27 9JJ. If the location of where the make-up/beauty/nail services is to be performed is within 15miles of the salon studio, NO travel costs are payable. Mileage above and beyond 15miles is charged at 40 pence per mile (round trip). Example: Hook to Basingstoke – 7.31 miles / Basingstoke to Hook – 7.31 miles FREE Hook to Winchester – 24.29 miles / 9.29 miles @ 40ppm = £3.72 Winchester to Hook – 24.29 miles / 9.29 miles @ 40ppm = £3.72 – Total Travel Cost £7.44 TRIALS. These take place at the salon studio in HOOK, HAMPSHIRE and are charged according to the price list. If travel is required by Azur Make-Up to perform the trial additional costs are incurred as stated above in “OTHER COSTS”. All trials are performed by Christina Phillips and a list of products used is recorded for each person having a trial (this can include bridesmaids, mother of bride/groom & other bridal party members). BOOKING. By completing & signing the booking form and returning it to Azur Make-Up, you are accepting Azur Make-Up’s offer to provide make-up/beauty/nail services on the date(s) given & agreeing to the Terms & Conditions. The booking form can be completed prior to the trial or at the date of the trial. DEPOSIT. A non-refundable deposit of 75% of the total booking is payable at the time of your trial to secure your booking. This will be deducted from the final balance. PAYMENT. All payments can be made either by cash, cheque and credit/debit card or via PAYPAL. The remaining balance is payable one week before the date of the make-up/beauty/nail service by either of these methods. Cheques should be made payable to Studio XN Ltd. CANCELLATIONS. A minimum of 7 days notice prior to the date is required for all cancellations. Cancellations less than 4 weeks notice prior to the date will still require the final balance to be paid. All cancellations must be made in writing followed by a phone call to confirm Azur Make-Up’s receipt of the letter. THE SERVICES. You (the client) are required to provide an accurate and complete description including any time restrictions of what you want and require on the day. Azur Make-Up reserves the right to supply an alternative make-up artist in the event Christina Phillips is not available. INSURANCE. Azur Make-Up is insured with BABTAC (British Association of Beauty Therapy and Cosmetology) to carry out all of the treatments and services advertised on leaflets and websites. In the event an alternative make-up artist is assigned to you on the day, Azur Make-Up can assure you the make-up artist is both qualified and suitably insured to carry out the service. MAKE-UP ARTISTS. The lead MUA is Christina Phillips, however in busy times additional MUA’s will be hired in to assist. Such MUA’s are freelance and will not be directly employed by Azur Make-up. In busy times Christina Phillips may not be able to provide the service and will outsource this to another MUA. Each MUA will be properly vetted to ensure they are qualified and insured to carry out make-up/beauty/nail services in accordance to the client’s request. The MUA’s are all paid by Azur Make-Up in accordance with what has been booked and paid for. If the client or other bridal party members request an extra make-up/beauty/nail services by the MUA Assistants on the day, it is the decision of the MUA Assistant if he/she wishes or has time to provide the additional service. Payment is made direct to the MUA Assistant.
