                        Consignment Agreement

This agreement is made between ______________________ (the consignor)
and Suzy Q’s Consignotopia Boutique. (Further referred to as Suzy Q’s)

The Consigner and Suzy Q’s acknowledge and agree that the Consigner has
provided goods described that meet all quality conditions listed below.
These items are placed for sale on a consignment basis, under the terms as
follows
    1. Consignor agrees to following:
       a. Clothing and/or items provided have no stains, holes, broken
           zippers, pilling, worn collar, loose or torn hems and seams.
       b. Items provided are freshly laundered or dry cleaned (if instructed
           by label on clothing), wrinkle free, odor free and pet hair and
           dander free.
       c. All items must be in one of the following conditions: New with
           tags, Nearly New, and/or Excellent condition.
       d. After the 60 day consignment period is over, all clothing becomes
           the property of Suzy Q’s and will be donated to charity **unless
           otherwise noted on contract at time of initial appointment.**
       e. Any consigned jewelry, designer handbags can be returned at
           the end of consignment period with not additional fee. These
           items must be picked up at the store location at the end
           contract date. If these items are not picked up within this time
           they become the property of Suzy Q’s Consignotopia.
       f. Due to administrative costs and increasing overhead Suzy Q’s
           is charging a $5 consignor and drop off fee per 60 day contract
           period. If consignor wants their unsold items returned there
           will be a $20 charge for returning of all items not sold.
    2. In effect to keep a higher caliber of inventory for my clientele, Suzy
       Q’s has the right to deny certain brands or brand name labels.
    3. Due to the large amount of work to have your clothes on consignment
       in this shop there will be a fee of $2 per item for retrieving your
       item(s) earlier then the end contract date.
  4. Suzy Q’s will determine the selling price of all items based on
     condition, brand, style, size and demand. If there is a minimum
     payout amount you would like it needs to be said and noted on
     item(s) at the appointment time.
  5. Suzy Q’s agrees to pay the Consignor 40% of the final sales price less
     taxes and/or additionally incurred fees. Incurred fees can be but not
     limited to repair to, dry cleaning, laundering and/or ironing to items
     given for consignment.
  6. Suzy Q’s agrees to use its best efforts to sell consigned goods for the
     best reasonable value for benefit of the Consignor. Suzy Q’s will
     account for sold items in payment form of check on the 20th- 25th
     of the month for item(s) sold in previous contract time. If
     payment is not picked up during the 20th-25th of the month you
     can call for a 30 day extension. If the monies have not been picked
     up after this time period your account balance will be taken to
     zero and your account will be closed. Upon payout if written check
     is lost or stolen the consignor will pay the cost to stop pay the check
     to have Suzy Q’s issue another check. The stop pay fee for Suzy’s to
     do this is $40.
  7. The selling of items for the consignment 60 day period are as follows:
         a. The first 30 days the item(s) are in full consignment price
             category.
         b. If the item has not sold within 31 days the item will/may be
             discounted up to 50%.
  8. Suzy Q’s is a clothing and accessories broker and can not buy
     insurance for inventory it does not own. Suzy Q’s agrees to take due
     care of items but can not be liable for theft, fire, and/or damage.

Suzy Q’s Consignments policy highlights:

   1.Please call in advance when you would like to
   pick up your monetary payout. The time this will be
   available will be between the 20th through 25th of the month at your
   end of contract date. Because of the overwhelming amount of
   paperwork needed to keep your account in order, a few days are needed
   to process it.

   2. Store credit is available as soon as your item(s) sell. You do not have
   to wait until end of 60 days to use credit accrued by items selling.

   3. If you can not make it at your end of contract specified date, you can
   call for a 30 day extension. What this means is your payout will fall to
   the next 20th through 25th day of the month. If monies have not been
   picked up after the 30 day extension, your account balance will be
   taken to zero and your account will be closed.

   4. If you are a consignor whose items ARE NOT donated, the store does
   not have enough space for storage. If your unsold items are not picked
   up by end of your 60 day consignment period, the items become the
   property of Suzy Q’s and they will be donated.
