HOW TO GET STATE LEGAL FORMS FROM THE INTERNET
STEP ONE: Type www.courtinfo.ca.gov/forms into the internet Address bar and press “enter”. STEP TWO: Screen A: Click on the arrow to bring out the drop down menu.

STEP THREE: Screen B: You have three options for getting a list of the forms: Form Number: If you know the number of the form you need, click on “All Forms Listed by Number”. Form Name: If you know the name of the form you need, click on “All Forms Listed by Name”. Form Group: If you are unsure about the form name or number, you can click on a group of forms (for example, “Civil”), to see all the forms for that group.

Click here to select a group of forms or to list forms by name or number.
Screen C STEP FOUR: Screen C: Click on “See Forms”. This will bring you to a list of the forms. STEP FIVE: Screen D: Scroll down until you find the form you are looking for, and then click on it. For most forms, you have two options: Non-Fillable: This will allow you to print a blank form and then fill it in by hand. Fillable: This will allow you to type in the information on the form and then print it out. NOTE: This website does NOT file your forms for you. In order to file your forms, you must either take them or mail them to the proper court and pay any required fees.
Scroll down until you see the form you need, then click on the form number under fillable or nonfillable (see explanation under Step Five for more information).

STEP ONE: Type www.sccsuperiorcourt.org into the internet Address bar and press “enter”. STEP TWO: Screen A: Click on “Forms” at the top of the page. STEP THREE: You have two options for listing the local forms: All Forms Listed by Name: Screen B: The page will automatically list all the forms alphabetically by name. If you know the form name, scroll down the list until you find it and then, click on the name of the form. The list also shows the form numbers and court division. (Note: If you click on the blue letters of the alphabet the page will jump to that part of the list.) You can either fill the form out online and then print it or you can print it and fill it in by hand.

Scroll down until you see the form you want and then click on the form name.

Click on the name of the court division.

Forms Listed by Division: Screen C: If you are unsure of the form name, you can scroll down to the bottom of the webpage and click on the name of the court division (ex. “Civil”, “Family”, “Juvenile” or “Probate”). This will bring you to a page that lists all of the local forms for that court division. (Note: Local Small Claims forms will be listed under Civil Division.) Screen D: Look at the list to find the form you need. Then, click on the number of the form. You can either fill the form out online and then print it or you can print it and fill it in by hand.
