Guests of CHS Students MUST make a copy of high school ID or photo ID (Driver’s License) and attached to this form.

By signing this CHS Prom Permission Form, I/we agree that the guest will follow all CHS rules for the 2012 Junior-Senior Prom. I/We understand
that if any rules are violated, the student and guest may be removed from the prom without a refund. As witnessed by the signatures above, I/we
understand that we have received, read, understand, and agree to the rules for the prom. I/We understand that I/we are responsible for the
behavior of the guest and are subject to the consequences of the actions of the guest.

NEED-TO-KNOWAND/OR PROVIDE
    Guests must present a High School ID or Drivers License at the prom sign-in table on April 9th.
    Guests must be under the age of 21 before April 14, 2012. No person under 15 or over 20 will be admitted,
     which also includes spouses of CHS students.
    Guests of CHS students are subject to all CHS rules and policies.
    Anyone going outside the Marriott building without permission will not be readmitted.
    If you buy a prom ticket and to not attend, there are no refunds.
    Guest Prom Permission Forms must be signed and approved by the CHS Principal or a CHS Administrator before a guest can attend.

CHS PROM DRESS CODES AND DANCE CODES
Females
       Slits on dresses must not go above the top of the knee
       Dresses no shorter than mid thigh
       No cutouts on dresses, no mid-riff showing, no dresses that expose sides
       Spaghetti straps and strapless dresses are acceptable
       Dresses with back open to waist or lower are not acceptable
       No see through dresses or suits
       Formal dress shoes, sandals, or flats are acceptable – no tennis shoes or flip flops

Males
       Dress shirts, tie, dress pants and belts are required (sport coat, suites, or tuxedoes are optional)
       Formal dress shoes – not tennis shoes or flip flops
       Jeans and/or golf shirts are not permitted
       No doo-rags of any type are allowed

DANCE CODE
       The 2012 Junior-Senior Prom is a formal off-campus event held at Disney EPCOT World Showplace Center. All activities throughout the
        evening (before and after prom) will reflect appropriate, ethical, and respectful behavior by all CHS students and guests. Any dancing
        showing explicit PDA behavior will result in a warning and possible removal from the Prom without a refund. We appreciate your
        corporation in making this prom a night to always remember!
