Credit Report Application Procedure

Required documentation: 1. Personal Credit Report Application – Form B: by Mail (blank form is available for download from JCIC website or by fax). 2. ID documents: Photocopies of residence permit and passport. Note on the photocopies “Identical to the original” and sign or affix seal on it. 3. Inquiry fee of NT$100 (cash only).
Required documentation: 1. Personal Credit Report Application – Form A: In Person (blank form is available at the JCIC counter). 2. Original residence permit. 3. Original passport. 4. Inquiry fee of NT$100 (cash only). ※To prevent the application of credit report using forged ID, JCIC will not accept the application if the originals of residence permit and passport are not presented at the counter.

Letter of authorization (can be download from JCIC website or drawn up and signed by the principal). 5. Inquiry fee of NT$100 (cash only). ※ Application by an agent who is not a relative of second degree or high of the principal can not be made for more than three times a year. Otherwise, JCIC will not accept the application.

The credit report will be mailed by express mail to the address indicated on the application.
Not waiting for pick-up or special case: The credit report will be mailed by express mail to the address indicated on the application.
Same-day pickup: Applicant waits for about 30 minutes to pick up the credit report with original residence permit or passport when his/her number is called.
