OUTSTANDING EDUCATOR AWARD
The nominee can be a classroom teacher, special area teacher, media specialist, reading
specialist or EEN teacher, etc. Honor your actively involved teacher by nominating
him/her for this award.
The criteria for this award is excellence in teaching effectiveness, past and present PTA
involvement as well as community and professional organizations involvement.
Awards for Elementary and Secondary (Pre-school, Middle/Junior High, and Senior
High) level will be presented at the annual Wisconsin PTA Convention. We would
appreciate that the winners be in attendance.
Note that this application is the only criteria used to determine our finalists. Take your time in completing this
application to best represent your nominee.
*If no candidate truly qualifies, no award will be given.
GUIDELINES:
1. Only 1 nomination can be made from each PTA unit.
2. PTA’s must complete the Nominee Information Form (page 3 of this form)
3. Selections Criteria must be submitted using no less than 12 POINT FONT. (page 2 of this form)
A minimum of 300 words is required for each question. No exceptions!
4. The Nominee Information Form must be signed by the Nominee and Local Unit President.
5. You may submit up to three (3) additional letters of recommendation to support the nomination. (Limited to
one (1) page each)
6. Facsimiles (faxed copies) and e-mailed nominations will not be accepted.
7. Mail 7 completed copies of the nomination to:

8. The local PTA must have the following items received in the WI PTA office by the appropriate deadlines.

● Applications and letters of reference will be not be returned.
● Make copies for your file.

SUGGESTIONS FOR SELECTION COMMITTEE
1. Committee of no less than 3 persons.
2. Committee may include parents, teachers, principal and former students, if applicable.
Input should be sought from a broad based group.
3. Committee should consider all questions on nominee form. Take time to answer
completely, examples are helpful.
SELECTION CRITERIA
Each criterion is followed by questions and suggestions for narrative. A minimum of 300 words is
required for each question. No exceptions!
1. Educator’s Relationship with Students. Describe how the educator interacts with students.
Be specific about such things as how the educator builds on student strengths, helps students
overcome weaknesses, recognizes and provides for individual differences among students,
encourages respect for differences, and inspires students academically and personally.
2. Educator’s Relationship with Parents. How does the educator support National PTA
National Standards for Family-School Partnerships? Describe how the educator
has initiated or supported communication between parents and students and between parents
and educators. How accessible is the educator to parents? Does the educator provide evaluations
of student progress and notify parents promptly of student achievements or problems? Are
parents involved and informed about decisions? Be specific and give examples.
3. Educator’s Relationship with PTA. Summarize the educator’s involvement with PTA.
How does he or she demonstrate commitment to the Objects of PTA?
4. Educator’s Relationship with the Community. Describe how the educator links the
classroom with the community. How does the educator involve community members in the life
of the school, draw on community resources, and encourage students to participate in the
community? Is the educator accessible to community leaders and other community members?
5. Educator’s Relationship with the Profession. Describe the nominee’s professional preparation
and participation in continuing education. Describe specific professional activities and
accomplishments such as development of curriculum, development of assessment processes,
sponsorship of student activities, mentoring of new students, and/or participation in professional
associations. Note any relevant honors.
