1. Please complete all sections of this form to ensure that the Core HR Team can arrange your interviews
to meet your requirements. Partially completed forms may result in delays or errors in interview

3. You are responsible for setting the interview date in line with the recruitment schedule agreed in
advance with your designated HR Officer. Please give us at least 2 weeks notice from receipt of this
form in advance of the interview date to allow us to arrange the interviews, invite candidates and

is G6/Ac1 or below . Please remember to notify all panel members about the interview arrangements. The
Core HR Team will send panel members an interview pack a few days prior to the interview.
