POSITION: Locum Consultant Physician in
          Acute Internal Medicine


The Department   Emergency and Medical Services Directorate

                 This encompasses the departments of Emergency Medicine, General
                 Internal Medicine (GIM) and its related specialties. There are 144 general
                 medical beds including a 6 bed coronary care unit and 6 bed high
                 dependency unit. The medical wards are specialty based. There are an
                 additional 100 care of the elderly beds including a stroke unit and geriatric
                 orthopaedic rehabilitation which are part of Lanarkshire’s Medicine for the
                 Elderly Directorate.

                 The Emergency Medicine Department is responsible for more than 75 000
                 emergency medicine attendances (of which 25% are children), 12 000 acute
                 admissions and 5 000 new outpatient referrals per year. Acute medical
                 admissions are admitted to the emergency care ward which admits all
                 medical admissions.      Patients are initially assessed and stabilised and
                 usually triaged within 24 hours to the appropriate specialty ward as far as is
                 practically possible.


                 Acute Internal Medicine (AIM)

                 The Medical Assessment Unit (MAU) admits approximately 12 000 new
                 patients per year. These patients are admitted to our 36 bedded medical
                 assessment unit and our 12 bedded combined coronary care and medical
                 high dependency unit. A small number of patients are admitted directly to
                 the stroke ward and adult critical care unit. All patients are assessed by the
                 on call physician on one of the twice daily post take ward rounds. Those
                 patients who are not discharged from MAU within 24-48 hours are triaged to
                 one of 7 specialty wards for continuing care depending on their clinical
                 needs. The acute receiving rota is 1 in 14 with 2 physicians responsible per
                 weekday and 1 each on Saturday and Sunday. On the weekdays, the ‘day
                 shift’ receives from 10pm till 6pm the following day and the ‘late shift’
                 receiving is from 4pm till 10pm with the added responsibility for telephone
                 advice overnight. The Care of the Elderly physicians now have dedicated
                 beds within the unit (presently 5) taking appropriate referrals from the
                 medical take directly into their service. In conjunction with our consultant
                 colleagues in the Emergency Medicine department, we are currently
                 developing alternative pathways for patients who may not require an acute
                 admission.

               The Wishaw Site Improvement Group has identified the development of
               Acute Internal Medicine as a key part of its work. It is planned to
               substantially increase the number of acute physicians to facilitate a number
               of proposed service developments detailed below. It is intended that once
               these posts are successfully filled that further new posts will be created as
               the next phase in the development of the specialty.

               Critical Care

               There is excellent 5-bedded Intensive Care Unit at Wishaw General
               Hospital, run by Intensivists / Anaesthetists, but with ongoing consultant
               physician input into the care of relevant patients. There is a combined 12
               bedded Coronary Care and medical HDU. Patients admitted to our
               Coronary Care unit are currently cared for by our 4 cardiologists Monday –
               Friday. With the development of the Optimal Reperfusion Service for
               Lanarkshire in 2008, nearly all patients with acute ST elevation MIs will be
               transferred directly to the Cardiac Catheterization Laboratory at Hairmyres
               Hospital for primary percutaneous coronary intervention. The medical HDU
               at Wishaw General is run by all the physicians. It is hoped with this
               appointment to take forward the restructuring of medical high dependency
               services.

POST INFORMATION
The Post     Locum Consultant Physician in Acute Internal Medicine based at Wishaw
             General Hospital.


General          You will be answerable to Clinical Director, who will agree your job plan.

DUTIES AND RESPONSIBILITIES
Main Duties   The post will be primarily based within the Medical Assessment Unit of
              the Hospital. The postholder will join an established group of physicians
              including 3 Consultant Physicians in Acute Internal Medicine in providing
              high quality clinical care for emergency medical admissions. You will
              play a lead role in the operational and strategic development of acute
              medical assessment in the hospital including the establishment of
              ambulatory care. You will have specific responsibilities to provide senior
              leadership in the medical assessment unit on specified clinical day. You
              will cooperate with colleagues and support an effective multidisciplinary
              team in the Acute Internal Medicine (AIM) Unit. You will work to ensure
              optimal use of medical assessment beds and contribute to efficient bed
              management through attendance at bed management meetings. You
              will be expected to lead and develop the interface between emergency
              medicine and acute medicine, breaking down traditional barriers. You
              will be a part of the critical care delivery group providing operational and
              strategic advice to the directorate and emergency medical division. You
              will share responsibility for plans to develop medical high dependency. It
              is anticipated that the new postholder will share responsible for raising
              the profile of Acute Internal Medicine within NHS Lanarkshire and more
              widely within the West of Scotland. The postholder will also be expected
              to participate fully in the clinical and educational supervision of junior
              medical staff with particular reference to training in acute medicine. You
              will contribute to protocol development, educational activities in relation
              to Acute Internal Medicine (AIM), and clinical governance and audit
              work. The post holder will contribute to the Acute Internal Medicine (AIM)
              Rota on an equitable basis with other colleagues (1 in 14 rota with
              prospective cover). An availability supplement is payable for out of hours
              work. In addition, the successful applicants will be responsible for a
              fixed dayshift per week of acute unselected medical receiving (8am to
              6pm).

                  The acute physicians are also expected to provide continuing care of a
                  small number of patients within the medical division undertaking 2
                  routine ward rounds per week and will contribute in rotation with
                  colleagues to 1 general medical out patient clinic per week.

TERMS AND CONDITIONS

a)   This appointment is offered on the Terms and Conditions of the Consultant Contract on
     a locum basis.

b)   The successful candidate will automatically be taken into membership of the National
     Health Service Superannuation Scheme unless he/she elects not to join.

c)   The employment is subject to 1 months’ notice on either side subject always to the
     appeal and other provisions of paragraphs 10.4 & 10.5 of the Terms and Conditions of
     Service of Hospital Medical and Dental Staff and Doctors in Public Health and the
     Community Health Service Consultant Grade.

d)   The successful candidate, if not already employed by the NHS Lanarkshire, will be
     required to complete a medical questionnaire to obtain medical clearance from the
     Occupational Health Physician.

e)   The successful candidate will be required to complete a disclosure Scotland PVG
     (Protecting Vulnerable Groups Scheme) form. No approach will be made without
     written permission of the successful applicant who will be asked to sign and complete a
     Disclosure PVG application, giving authorization for the check to be undertaken.

f)   NHS Lanarkshire does not negotiate salary placements. On commencement the salary
     will be in line with paragraph 11.1.2 of the terms and conditions of the new consultant
     contract. Appointees start on the scale minimum except in the circumstances of
     paragraphs 11.1.3 – 11.1.4 of the terms and conditions of service.

g)   From 1st April 2012 the starting salary for the post is £74,504 per annum (based on 10
     Programmed Activities per week). The successful candidate’s total salary will be
     dependent on his/her previous service history. Remuneration for any extra
     programmed activities will be dependent upon the job plan agreed at the time of
     appointment.

Registration with the General Medical Council or General Dental Council imposes on

IMMIGRATION
Overseas        For the purpose of immigration status, overseas doctors and dentists are
