Dear Dog Owner, Thank you for your recent inquiry about do daycare. Las Vegas Zen Dog is committed to providing a safe, fun, and stimulating social environment for you pet during weekday business hours. At dog daycare, you pet will not be crated for long hours, but will enjoy supervised playtime with other pets and our friendly staff. Enclosed, you will find information and forms you need to register your pet for our services. There is a one-time, $25.00 non-refundable application processing fee for each pet. To enroll, simply fill out the enclosed forms and return them to Las Vegas Zen Dog along with proof of vaccinations and check or money order for $25.00 for each pet. You many enroll through the mail, via internet (www.lasvegaszendog.com) or if you prefer, you may drop your application off in person. Once we receive your enrollment form, proof of vaccinations and enrollment fee, we will screen your paperwork and call to schedule a time when we can meet your pet. Dog daycare General Information and Policies The purpose of Las Vegas Zen Dog is to provide a safe, fun, and stimulating social environment for dogs during weekday business hours. To ensure the safety and health of you pet and our other guests, we require all guests to comply with the following rules and regulations: SEX: All dogs 6 months old or older must be spayed or neutered. SHOTS: All dogs must have up-to-date vaccinations. Owners must submit written proof of DHLPP, Rabies, and Bordetella vaccinations. HEATH: All dogs must be in good health. Owners will certify their dogs(s) are in good health and have not been ill with a communicable condition in the last 14 days. Upon admission, all dogs must be free from any condition which could potentially jeopardize other guests. Dogs that have been ill with a communicable condition in the last 14 days will require a veterinarian certification of heath to be admitted or readmitted. MEDICATIONS: All medication to be administered during our care must be supplied by owner. Containers must be clearly marked with dog’s name, medication, dosage, and reason for medication. EMERGENCIES: In case of emergency we will contact you. We will use your veterinarian if possible or the nearest veterinary emergency hospital. BEHAVIOR All dogs must be non-aggressive and not food or toy protective. Owners will certify their dogs have not harmed or shown any aggressive or threatening behavior towards any person or any other dog. Please remember: your pet will be spending time with other pets and the safety and health of ALL animals is paramount. Las Vegas Zen Dog reserves the right to refuse or to reject any dog that might be a safety hazard to itself, other dogs in our care or staff members. FOOD: All dogs being fed in our care are required to have their own food. Please provide written feeding instructions on the bag. APPLICATION: All dogs must have a complete, up-to-date and approved application on file. There is a one-time $25.00 non-refundable application screening fee for each pet. DAYS AND HOURS: Monday through Friday from 6:30am to 7:00 pm. Saturday and Sunday 7:00 to 6:00 pm. Las Vegas Zen Dog is not an overnight facility. There is a $1.00 per minute late charge if picking up after business hours. RESERVATIONS: Reservations are required. Cancellations with less than 24 hours notice will be charged full fees.

I understand that I am solely responsible for any harm or damages caused by my dog(s) while my dog(s) is/are attending Las Vegas Zen Dog and agree to pay for said harm or damages in full. 2. I understand and agree that in admitting my dog(s) to Las Vegas Zen Dog, staff have relied on my representation that my dog(s) is/are in good health and has not harmed or shown aggressive or threatening behavior towards any person or any dog. 3. I understand and agree that Las Vegas Zen Dog and their staff, will not be liable for any problems that develop, provided reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever arising from my dog’s attendance and participation in day care. 4. I understand and agree that dogs can sometimes receive minor cuts and scratches at daycare and any problems that develop with my dog(s) will be treated as deemed best by staff and volunteers of Las Vegas Zen Dog, at their sole discretion, and that I assume full financial responsibility for any and all expenses involved. 5. I understand that Las Vegas Zen Dog shall exercise reasonable care for my dog(s). It is expressly agreed by owner that Las Vegas Zen Dog’s liability shall in no event exceed the lesser of the current chattel value of my dog(s) or the sum of $1000.00 per dog. 6. I understand and agree that if my dog is not picked up by the end of the business day, Las Vegas Zen Dog is authorized to take whatever action is deemed necessary for the continuing care of my dog, and I agree to pay for any costs associated with continued care.
