Membership Renewal letter 2012

Dear Parent/Member,

Please find enclosed your annual Club membership pack for 2012.

All members are obliged to join/renew their membership with the Club and the ASA accordingly.
Your membership and payment must be received by the 27th January 2012 in full to avoid Club and
ASA memberships being lapsed. Please bear in mind that if you do not register you/your child will
not be insured to swim as part of their squad or within a competitive event. Please do not pass your
registration to anyone other than the Membership Secretary.

Please complete the enclosed membership form and contact/medical form. This is a Club
requirement in order that we can ensure contact data is up to date in case of emergency.
If you are new to the Club then you must also complete an ASA membership form.

Please submit your payment and completed paperwork at the allocated session below:

Registration dates for squads:

Performance A & B/Junior Squads         Monday 23rd January 2012: 5.00pm -7.30pm

Development A                           Tuesday 24th January 2012: 5.30pm – 8.00pm

Development B                           Thursday 26th January 2012: 5.30pm-7.30pm

Performance A & B/Junior Squads         Friday 27th January 2012: 4.30pm-8.00pm

Masters/Dynamo/Water Polo/Diving: Please submit paperwork on one of the above dates.

Please ensure that you have completed your form fully in line with the enclosed guidelines. All
payments to be made by cheque to COSASC with the members full name on the back and squad.
Spare copies of paperwork are not available at these sessions. No late entries will be accepted after
27th January.
