COE Installation Quality Bulletin
Installation Quality Bulletin Number: 09-06
To: All Installation Suppliers / Central Office Operations Representatives / Design Engineering Representatives / Quality Managers / Field Engineering Representatives / AMC Representatives Date: Thursday, July 9, 2009 From: QWEST Installation Quality Assurance Subject: Pentair™ Cabinet Caster Dolly Assembly Return Policy.
Action Required: NO: YES: Description of Bulletin: The purpose of this Bulletin is to provide Qwest-approved COE installation Service Suppliers with instructions necessary to return the Pentair Central Office cabinet base caster dolly assemblies. All new 3’-0” and 4’-0” deep Pentair cabinets come equipped from the manufacturer with a caster dolly assembly attached to the front and rear of the cabinet base in order to easily maneuver the cabinet into the proper position prior to anchoring. Empty caster dolly return boxes (Refer to Figure 1) will be provided with each new Pentair Central Office cabinet. After removal of the caster dollies from the cabinet, the Service Supplier will place both front and rear caster dollies in the return box provided. Note: As of 7/1/09, Service Suppliers are required to use the return boxes to return all Pentair cabinet caster dolly assemblies. Under no circumstances will caster dollies be discarded.
Caster Dolly Return Box and Return Label
Figure 1 depicts the labeling provided on the caster dolly return boxes. In the lower left corner of the photo, the UPS shipping return label is visible. The UPS return label is already attached to the box so it will be easier for the Service Supplier’s installer to ship the caster dollies back to Graybar Electric in Denver, Colorado. No additional labeling is necessary.

There are only two Qwest-approved methods to ship back the detached Pentair cabinet caster dolly packages. Service Suppliers must utilize either UPS or Graybar Electric. UPS can be contacted directly to come to the site and pick up the caster dolly package or the packages can be dropped off at the nearest UPS shipping facility. As an alternative, Service Suppliers in the Denver, Colorado metro area may elect to drop off the caster dolly assembly packages directly to a Graybar Electric facility. No other drop off locations are authorized or approved. Acknowledgments to Diana Unser (AMC) for her contribution to the content of this Bulletin.
