   SECOND DEGREE APPLICATION FOR ADMISSION TO SEMESTER 1- CLINICAL NURSING -
                                SUMMER 2012

GENERAL DIRECTIONS AND CHECKLIST: ALL OF THE FOLLOWING MUST BE COMPLETED TO BE
CONSIDERED FOR THE SECOND DEGREE CLINICAL NURSING PROGRAM.

□1.        I HAVE BEEN ADMITTED TO THE UNIVERSITY.
            In order to be screened for admission to the clinical nursing major of the School of Nursing, you must first be admitted to
            the University of Northern Colorado.
           •    If you have never attended the University of Northern Colorado, you must apply to the University as well as to the
                School of Nursing. Formal applications to the University are obtained from and returned to the UNC Admissions
                office. They are also available on the www.unco.edu website.
           •    If you are presently enrolled at the University of Northern Colorado or were previously enrolled and withdrew in
                good academic standing, you need only apply to the School of Nursing.
           •    In order to avoid paying Graduate tuition, Second Degree students may need to fill out a “Change of Major” form in
                the School of Nursing office listing “Pre-Nursing” as a second degree.

□ 2.       I HAVE COMPLETED THE SCHOOL OF NURSING (SON) CLINICAL APPLICATION.
           •   Applications for the Clinical Nursing Program with all required attachments (see checklist) should be submitted on or
               after January 4, 2012 to the School of Nursing in Gunter 3080, or mailed to: UNC, School of Nursing, Attention:
               Undergraduate Applications, Gunter Hall 3080, Box 125, Greeley, CO 80639. Repeat applicants must submit
               all required documents, including official transcripts, even if turned in previously. All completed School of Nursing
               applications must be:

                   Received in the School of Nursing office in Gunter 3080 on or before January 25, 2012 at 5 pm.

□ 3.       I HAVE ATTACHED/SUBMITTED ALL MY OFFICIAL TRANSCRIPTS.
           Students must submit OFFICIAL transcripts to document the following:
           • Last 45 hours of course work
           • All completed nursing prerequisite courses
           • Previous degree(s) awarded
           • Be sure fall semester grades are included with transcripts.

           Guidelines for submitting official transcripts
           • If the school issuing the transcript will send the transcript directly to you:
               o Open the transcript.
               o Highlight the nursing prerequisite courses or equivalencies and degree(s) awarded.
               o Staple the transcript to your application.
               o Submit to the School of Nursing. (We can tell that it is an official transcript by the seal.)
               o Do this even if the envelope states, “This transcript is not official if seal is broken.”
           • If the school will not issue the official transcript directly to you, then have it mailed to:
               o UNC School of Nursing, Attention: Undergraduate Applications, Gunter Hall 3080, Box 125, Greeley, CO
                   80639.
               o School of Nursing must be in the address for the transcript to be sent to the SON office.

□ 4.       I HAVE ATTACHED PROOF OF CURRENT CNA.
           A Certified Nurse Assistant class must be completed by January 25, 2012. Attach a copy of CNA license or proof of
           CNA course completion to your application. Official certification is required by May 1, 2012. Out-of-state CNA
           certification is acceptable.

SCHOOL OF NURSING ADMISSION POLICY—CRITERIA FOR ADMISSION

All students who have a previous Bachelor’s Degree as recognized by the UNC Admissions Office and meet minimum
requirements for admission to the clinical level of the School of Nursing will be considered. Be sure to document proof of
degree(s) awarded.

    1.   Admission to the University of Northern Colorado.

    2.   Completion and verification of Certified Nursing Assistant course or certification.

    3.   Potential for completion of all required prerequisite courses by the end of Spring Semester, 2012.

    4.   Copies of official transcripts must be attached to your application.

         a.       A minimum cumulative grade point average of 3.0 on a 4.0 scale of most recent 45 credit hours.

         b.       Only students who have a grade of C or better (C- is not acceptable) in each of the prerequisite
                  courses or course equivalent will be considered for admission. If a student is admitted to the
                  Nursing program and earns less than a grade of C in any of the prerequisite courses or course
                  equivalent, their admission to the Nursing Program will be rescinded

         c.       Computation of Grade Point Average
                     • GPA for screening purposes will be calculated using the most recent 45 semester hours of
                         coursework from Fall semester back and GPA from all of the completed nursing prerequisite
                         courses.
                     • High School Advanced Placement scores are not factored into GPA.
                     • Grades from technical courses such as CNA, EMT etc. will not be included.



IF ACCEPTED INTO THE CLINICAL NURSING PROGRAM

    1.   NOTIFICATION - Notification letters will be mailed to applicants in March 2012.

    2.   REQUIREMENTS AND INFORMATION FOR NEW CLINICAL STUDENTS - Information regarding
         admission requirements such as purchase of malpractice insurance, specific health clearance requirements,
         CPR certification, purchase of uniforms and ATI materials, and registration and rotation plans will be mailed
         to admitted students once they have indicated they plan to attend the UNC Nursing program.

    3.   An optional meeting for all newly admitted nursing students will be held in late March to answer any questions about
         Nursing Program requirements.

FOR YOUR INFORMATION— EXAMINATION FOR REGISTERED NURSE LICENSURE

Upon completion of the nursing program you become eligible to take the NCLEX, a National Board of Nursing
licensing exam for the Registered Nurse license. The following questions are asked on the application for licensure.
If you answer “yes” to any of them, you may not be eligible for licensure.

        •   Has any nursing or other health care license held by you ever been denied, revoked, suspended,
            reprimanded, fined, surrendered, limited, or placed on probation in any state other than Colorado or in
            any territory of the United States?

        •   Are you under investigation or is a disciplinary action pending against your nursing license or other
            health care license in any state or territory of the United States?

        •   Have you received notification from the Department of Health and Human Services, Office of the
            Inspector General, that you have been excluded from participation in Medicare, Medicaid or any federal
            health care programs based on program related crimes and discipline?

        •   Have you ever been convicted, entered a plea of guilty, nolo contendere, or no contest for any felony,
            misdemeanor or petty offense?

        •   Have you ever been convicted, pled no contest/nolo contendere, or had a court accept a plea to a
            criminal motor vehicle offense of DUI/DWI/DWAI/OWI or any traffic offense involving drugs or alcohol?

        •   Has any final judgment, settlement or arbitration award for malpractice been paid by you or on your
            behalf?

        •   In the last five years, have you been diagnosed with or treated for a condition that significantly disturbs
            your cognition, behavior, or motor function, and that may impair your ability to practice as a professional
            nurse safely and competently, such as bipolar disorder, severe major depression, schizophrenia or other
            major psychotic disorder, a neurological illness, or sleep disorder?

        •   Do you now abuse or excessively use, or have you in the last five years abused or excessively used,
            any habit forming drug, including alcohol, or any controlled substance that has
            a) resulted in any accusation or discipline for misconduct, unreliability, neglect of work, or failure to
               meet professional responsibilities; or
            b) affected your ability to practice as a professional nurse safely and competently?

        •   Have you been terminated or permitted to resign in lieu of termination from a nursing or other health
            care position because of your use of alcohol or use of any controlled substance, habit-forming drug,
            prescription medication, or drugs having similar effects?

        •   Have you been arrested for an alcohol or drug-related offense other than stated in bulleted item No. 5?

You should also be aware that the School of Nursing is required to report knowledge of any of the above behaviors
to the Colorado State Board of Nursing. Although an affirmative answer to one of these questions does not
automatically mean you will be denied licensure, it may mean the Board will not consider you eligible even if you
pass the NCLEX exam. Each case is considered on its individual merit.
