Release of Liability for Alleged Breach of Employment Contract by Employer
This Release is made and entered into on this (date), by (Name of Employer), a
corporation organized and existing under the laws of the state of (name of state), with
its principal office located at (street address, city, state, zip code), referred to as
Company or Releasee, of (street address, city, state, zip code) and (Name of
Employee), hereinafter called Releasor of (street address, city, state, zip code).
WHEREAS, a certain Contract of Employment was entered into between Releasor and
Releasee on (date), and a copy of the Contract is attached hereto as Exhibit A and
made a part hereof; and
WHEREAS, Releasor asserts that Company has breached the Contract in that (briefly
describe nature of breach), and that, as a result of such breach, Releasor has suffered
damages in the total amount of $______________, itemized as follows: (e.g. loss of
wages, mental distress, etc.); and
WHEREAS, Company asserts that it has not breached the contract or, in the alternative,
that if it has breached the contract, damages sustained by Releasor
