This Release is made and entered into on this (date), by (Name of Employer), a

WHEREAS, a certain Contract of Employment was entered into between Releasor and

made a part hereof; and

WHEREAS, Releasor asserts that Company has breached the Contract in that (briefly
describe nature of breach), and that, as a result of such breach, Releasor has suffered

wages, mental distress, etc.); and

WHEREAS, Company asserts that it has not breached the contract or, in the alternative,
