An affidavit is a formal sworn statement of fact, signed by the person making the statement (who is called the affiant), and witnessed by a taker of oaths, such as a notary public. The notary public (or other takers of oaths) gives a statement regarding the veracity of the affiant's signature. The following form is an affidavit in which the affiant is verifying a claim that is described above the verification.
