Format for Excuse Letter
Criteria:
1. Formal Letter. 2. Date of absent. 3. Your Name / Class / Section must be stated. 4. Parent full name / IC number / Contact number / Signature. 5. All reason must be Explained in full e.g. It’s not approved when you just write: Home got things / Sick. Approved: There is a fire broke out in my house / I am sick due to a fever. 6. Sickness must be covered with an M.C. (Medical Certificate) (Attached) otherwise, inform your Sectional leader on the day / or the day before if under home-medication. 7. Passport must be photocopied if you’re overseas on the day of the practice. 8. Death Certificate must be attached when required 9. If there is a family function or there are family activities on the day of practice, inform your Sectional leader beforehand; not the last minute, and wait for the teacher-in-charge’s approval. 10. All letters must be submitted by the next CO practice.
Example Letter on Sickness: Example 1: Dear Teacher-in-charge,
I was absent from CO (Chinese Orchestra) practice on Date Absent Stated as I am down with Illness Stated. Please excuse me of any activities on that day.
Parent Acknowledgment: I, Parent Name ( Parent IC ), Mother/Father/Guardian of Child’s / Ward’s Name acknowledge that my child is sick and cannot attend Chinese Orchestra Practice on the date stated above. *Medical certificate Attached

Example 2: Dear Teacher-in-charge,
