The undersigned parent(s) and or guardian(s) of (name of child), hereby give permission for
(name of child), my/our minor child to attend and participate in all classes and activities of (Name
of Studio), a corporation organized and existing under the laws of the state of (name of state),
with its principal office located at (street address, city, state, zip code), referred to herein as
Studio. As a condition of (name of child) being allowed to attend and participate in such classes
and activities, I hereby release Studio, its directors, agents, officers, and employees from any
liability which may or could occur by reason of any personal injury or property damage suffered
by me or (name of child), regardless of the cause or alleged cause of such personal injury or
property damage.

The undersigned understand(s) that (name of child) will be voluntarily participating in activities
(e.g., including, but not limited to dance instruction, weight lifting, and/or strenuous aerobic
exercise) which may expose him/her to some level of risk or injury and the undersigned
represent(s) that I/we are aware of the nature of these activities and agree to accept any and all
risks associated with the participation of (name of child) in these activities.

The undersigned represent(s) that (name of child) is in good physical health and that unless I/we
have notified Studio in writing that (name of child) is unable to participate in an activity due to
