							Blood Transfusion
Medicine
      Blood products
Any therapeutic material which is
produced by human blood
   Blood components are
  Separated by differential
centrifugation and by freezing
         and thawing
 Advantages of use of
blood components and
    blood products
Can treat several patients with one unit
of blood
Reduce the exposure to unnecessary
antigens
Volume overload is less
   Blood components
Seperated by differential centrifugation
and freezing and thawing.
Packed cells FFP
Platelets Cryo precipitate
Buffy coat Cryo poor plasma
Need for grouping and
  cross matching
All plasma products need to be group
compatible
All products that contain significant
amounts of red cells need cross
matching e.g. Red cells, buffy coat
    Packed red cells
Stored at 4-6°C for 35 days.
Indication: anaemia
Dosage: 4ml/kg raises Hb by 1g/dl
Group compatible and cross matched.
    Washed red cells
Used as a method of depleting white
cells
Useful in patients with febrile
transfusion reactions
Once prepared it should be transfused
within few hours
Platelet concentrates
Stored at 22°C on a platelet agitator for 5 days.
Indication: thrombocytopenia, massive
transfusion, DIC
Blood group should be ABO compatible
Types of preparations:
Single donor apharesis: 240 x 109/l
Recovered platelets from random donor:55 x 109/l
Dosage: 4-6 single random donor concentrates
(1/10 kg body weight)
Each platelet concentrate increases platelet count
by 5-7.
   Platelets contd……
Platelets take 4 hour for full function
HLA matched platelets (if recovery in 15
minutes is poor.
Cut off point depends on the condition
Not indicated for stable ITP pts with plt
count over 10
Contraindicated in TTP & heparin
induced thrombocytopenia
        Buffy coats
Stored at room t°for 8 hrs only.
Indication: granulocytopenia
ABO and Rh group compatible, cross
matched
 Fresh frozen plasma
Stored at –20 °C for 1 year
Indications: Haemophilia B
             Warfarin over dosage
             DIC
             Massive transfusion
ABO incompatible
Dosage: 4 units of FFP
     Cryo precipitate
Produced by freezing and thawing of
FFP
Contains fibrinogen, FVIII, VWF
ABO compatible
Indications: Haemophilia A
              VWD
              DIC
              Massive transfusions
      Stored plasma
Stored at –20 °C for 1 year
Indications: Hypo protinaemia
             Burns
 Contains plasma proteins
      Blood products
Factor VIII concentrate
Factor IX concenctrate
Albumin
Human immunoglobulin
Hyper immune sera
