Educational Assistance Program for Employees
Whereas, (Name of Employer), hereinafter called the Employer, has established an
educational program to help eligible Employees develop their skills and upgrade their
performance; and
Whereas, educational assistance is provided for courses offered by approved
institutions of learning, such as accredited colleges, universities, and technical colleges
or schools that offer certification in a field approved by Employer; and
Whereas, courses must be, in the opinion of Employer, directly or reasonably related to
Employee’s present job or part of a degree program, or in line with a position that
Employer believes Employee can reasonably achieve; and
Whereas, all full-time regular Employees who have completed a minimum of one year
of service are eligible to participate in the program; and
Whereas, the undersigned Employee desires to participate in such program;
NOW, THEREFORE, Employer and Employee agree as follows:
A. Employee desires to enroll or is enrolled in the following school or technical
college and is pursuing the following described degree or certification:
B. Employee agrees that pursuing said degree or certification will not interfere with
job responsibilities of Employee.
C. Reimbursement covers actual costs of tuition and registration fees only and is
limited to a maximum of (number) credits per semester for approved courses, based
on the following schedule:
