MESSAGE FROM THE CHAIR


                              At Donate Life Connecticut, we continue to feel
                              incredible gratitude for our community of support. Your
                              time, passion and financial contributions allow the
                              organization to continue to grow and inspire more state
                              residents to register as organ and tissue donors. With
                              your help, we are expanding current programs and
                              exploring new opportunities to share the importance of
                              donation.

                               Currently, close to 100 million people in the United
                               States are registered donors and states are working hard
                               to determine how to keep improving that number in order
                               to save and improve more lives. As the local affiliate of
                               Donate Life America, Donate Life CT is invested in
practicing the best ways to encourage more people to donate. We are dedicated to our
work with the Department of Motor Vehicles, high school students, and state and local
officials and bring national programs to the state—like Flags Across America. Donate
Life CT participates in national meetings and works closely with the state’s donation
and transplant community to ensure that our mission is met.

As we continue to expand our outreach, we are eternally grateful for the way you have
carried our mission with you. Your financial support has funded new projects and im-
proved our signature offerings, while your time and your personal stories have encour-
aged many people to join the Donor Registry.

As the call to “Donate Life” canvases the state, we look inward with appreciation to
you…our devoted community of volunteers, donors and supporters.
              DEPARTMENT OF MOTOR VEHICLE OUTREACH
The partnership between Donate Life Connecticut and the Connecticut Department of
Motor Vehicles (DMV) has continued to grow. Each branch of the DMV has at least
one Donate Life volunteer visiting monthly to show appreciation and encourage the
employees’ efforts to ask ‘every customer, every time’ if they would be interested in
joining the State Donor Registry. With LifeChoice Donor Services, the New England
Organ Bank and the Connecticut Eye Bank, Donate Life Connecticut customized
inserts to be included in every license renewal form mailed from the DMV. The
local story highlights five year old Aaron of Seymour. Aaron received a heart transplant
in 2007.
UNIVERSITY OF BRIDGEPORT BASKETBALL
Stephanie Del Preore joined the
University of Bridgeport community
as the head Women's Basketball
Coach at the end of 2010. Coach
Del Preore reached out to Donate
Life Connecticut to coordinate a
Donate Life Day with the Men's
and Women's Basketball Teams in
January 2011 in honor of her late
father whose life was extended 6
years with a double organ
transplant. Del Preore's athletes
wore green bracelets and Donate
Life tees, raised over a thousand
dollars and registered nearly a
dozen new donors!




VOLUNTEER SUMMIT (APPRECIATION & TRAINING)
                                                     Thirty-nine Donate Life volunteers
                                                     spent half a day at J. Christian's
                                                     Restaurant in Wallingford learning
                                                     how to better communicate personal
                                                     donation and transplantation stories
                                                     to best impact the public. Attendees
                                                     of this Saturday afternoon in March
                                                     enjoyed a delicious, hot lunch and
                                                     left with Donate Life garden flags to
                                                     display in their home gardens. One
                                                     attendee shared his reflection of the
                                                     event: “To be honest, I'm not big on
                                                     seminars, trainings, and whatnot. I
                                                     want to tell you how much I enjoyed
                                                     the training on Saturday. Great
                                                     place, very good content, the guide-
lines were specific and relevant, and I forced myself to think about what I would say in
4 minutes, which is an important task. I especially liked the emphasis on participants’
input formally and informally, as opposed to dry lecture. I found the audience
delightfully interested in the topic and a real sense of working together.”
                                  FIREFIGHTERS GOING FURTHER

The Westport Fire Department led by
Firefighter Joe Arnson initiated a
program to bring donation education to
first responders around the state. The
program kicked off at a flag raising
ceremony held at the Westport Fire
Headquarters and was attended by
United States Congressman Jim Himes.
Other speakers included Westport First
Selectman Gordon Joseloff, double lung
recipient and Maine firefighter, George
Carman, local Westport heart recipient
Adrian     Pace   and      Donate    Life
Connecticut Director, Kari Mull. The first
responder outreach program, dubbed
“Fire Fighters Going Further” plans to
educate each fire department town by
town.
       NATIONAL DONATE LIFE MONTH
       Donate Life Connecticut kicked off National Donate Life Month with our Annual
       Celebration event at the State Capitol. Event speakers included Non-Profit Liason
       to the Governor, Deborah Heinrich, Department of Motor Vehicle Commissioner
       Currey, and Representative Labriola. Inspirational speaker, Dr. Tom Dempsey, shared
       his life-saving experience of receiving a liver transplant, a story he shared regularly
       with his patients. One such patient was young Jennifer who, following an appointment,
       told her mother, Lorraine, she wanted to be a donor when she passed away. When
       Jennifer unexpectedly passed away less than a year later, Lorraine remembered this
       conversation and Jennifer became an organ and tissue donor, saving the lives of
       dozens of people.

       ANNUAL HIGH SCHOOL POSTER CONTEST
                                           The 21st Annual High School Poster
                                           Contest drew over 200 submissions from
                                            20 high schools throughout Connecticut.
                                            The 2011 Awards Ceremony followed
                                             the annual kick-off to National Donate
                                             Life Month at the State Capitol in
                                              Hartford.  Each year, Donate Life
                                                Connecticut invites all Connecticut
                                &T ony Award    high school students to submit

STORY OF HOPE




The Barons honored their son Bruce’s, decision to become
an organ and tissue donor when he unexpectedly passed
away. Tom received Bruce’s liver in 1996. Tom lives a full
life with a beautiful family and a busy optometry practice.

One afternoon while treating a young female patient, Tom
found himself recounting the story of his transplant. So
struck by her doctor’s medical history, twelve-year-old
Jennifer declared she wanted to be an organ and tissue
donor, on the ride home from the appointment. That
conversation with her young daughter would come flashing
back months later when Lorraine learned the news her
daughter was brain dead following a church snow-sledding
accident. Lorraine honored her daughter’s wish and
Jennifer became a donor helping dozens of others.

Inspired by her niece, Janlyn felt she had something to
give. She heard of an acquaintance that was in
need of a kidney transplant. Janlyn decided to be
tested for his match. In December of 2006, Janlyn
donated one of her kidneys to her now, good
friend, Rob.

Tom, Lorraine, Janlyn and Rob joined Donate Life
Connecticut at the State Capitol to celebrate
National Donate Life Month.
                                      FLAGS ACROSS AMERICA
                                       Sixty-four cities and towns across
                                       Connecticut participated in the national
                                       initiative, Flags Across America by flying a
                                       Donate Life Flag during April, National
                                       Donate Life Month. This program honors
                                       those that gave the gift of life through
                                       donation, celebrates the lives saved and
                                       brings awareness to those that still wait.

            DIVINCENZO BOWL FOR LIFE
The DiVincenzo family celebrates
the new lease on life their young
son, DJ, was given with a heart
transplant at the age of 6 months
old.   The family held their 2nd
Annual Bowl for Life at the Milford
Lanes this past May! Over 25 lanes
were full of        bowlers vying for
prizes and awards! The event was
a huge success and a ton of fun for
all ages - 4 to 84!




     NOVAK GOLF TOURNAMENT
                             The 3rd Annual Rob Novak
                             Golf Tournament was held at
                             the Lyman Orchards this
                             past June honoring the life of
                             Rob     Novak,    a    young
                             husband, father and organ
                             donor. Donate Life CT is
                             appreciative of the Novak
                             family for donating a portion
                             of the proceeds for public
                             outreach.

                             Greg Novak, Rob’s brother,
                             has created a memorial
                             video highlighting the gift of
                             organ and tissue donation.
               3RD ANNUAL TOAST TO LIFE
           Donate Life Connecticut held the 3rd Annual
           Toast to Life event at Gouveia Vineyards in June.
           Guests were treated to a beautiful sunset while
           sipping on delicious wines. The sold out event
           was attended by 200 Donate Life supporters who
           helped the event to raise over $24,000. Thank
           you to all that attended or contributed items to the
           fabulous silent auction. Special awards were
           given to the excellent transplant surgeons we
           work with in Connecticut: Dr. Sukru Emre (YNHH)
           and Dr. David Hull (HH). The event was emceed
           by Channel 8 news anchor, Kristen Cusato.

DMV AMBASSADOR & OUTREACH PROGRAM




In July, Donate Life Connecticut began a wonderful new relationship with the
Department of Motor Vehicles (DMV). The new DMV Commissioner, Melody Curry, is
extremely supportive of donation, encouraging us to interact with her employees -
allowing us to thank them for asking every customer, every time to join the Donor
Registry. Without the DMV employees, it would have been very difficult to have
reached one million registered Connecticut Donors at the end of 2010. This program
is in partnership with LifeChoice Donor Services and the New England Organ Bank.
            ING HARTFORD MARATHON OFFICIAL CHARITY




Donate Life Connecticut was chosen as an Official Charity of the 2011 Hartford ING
Marathon (half, relay and 5K). Our organization was able to provide the marathon with
39 volunteers to help organize the start of the race! We had 28 runners run with us -
and together we raised over $8,000!
ROCK AND ROLL
Rock and roll icon, Ronnie Spector, coordinated a benefit concert with fellow stars
(John Cafferty & the Beaver Brown Band, Christine Ohlman, Michael Carabello of
Santana, The Uptown Horns, Dennis Diken of The Smithereens) in Collinsville,
Connecticut. Donate Life Connecticut hosted an awareness table at this November
event to benefit UNOS (United Network of Organ Sharing). Spector's second
husband is a kidney recipient. The couple lives in Connecticut.
                                                 THE POWER OF GIVING




The Power of Giving segment on Connecticut Public Broadcasting’s 'All Things
Connecticut' contacted Donate Life Connecticut to profile the organization and its
mission to raise donor awareness. Volunteer, Dara Brooks was interviewed and her
family's story of life was profiled on the show in December. Dara’s infant son was
diagnosed with biliary atresia and was in need of a liver transplant. Dara became the
first related living donor at John Hopkins Hospital when she donated a portion of her
liver to her son. Unfortunately, when Tyler passed away a little while later, Dara and
her family had the strength to consent to organ and tissue donation. Tyler saved the
lives of many others, including a female infant who had been hospitalized with him at
the same time. The woman is now nearly 20 years post heart transplant and doing
well.
OTHER HIGHLIGHTS


                     Donate Life Connecticut was the December beneficiary of a local
                     discount services website. Shoreline Perks donates 5% of
                     monthly proceeds to a charity. Donate Life Connecticut received
                     a check for $250.




Board Member, Zvi Cole took the Donate Life message on his back when he
participated in the 42 mile NYC 5 Boro Bike Tour in May! Zvi raised nearly $4,000 for
Donate Life Connecticut.



A special thanks to Patty Shaw and friends for
holding the 2nd Annual Billiard Event to raise
funds and awareness for organ and tissue
donation. Donate Life Connecticut is one of the
beneficiaries.

Nothing speaks louder for our organizational work than the number of dedicated
VOLUNTEERS that share our message to the public. Seventy-five individuals gave
their time to further organ and tissue donation awareness in Connecticut.

decision can make in the lives of so many others.

Creating donation friendly communities around the state led to increased donor aware-
ness. This year, 47 municipalities raised a Donate Life Flag outside town hall to call
awareness to National Donate Life Month.

DONATE LIFE CONNECTICUT’S IMPACT
Donate Life Connecticut is working to address the critical need for organ and tissue
donors through public outreach. In 2011, efforts of the organization contributed to
more than 36,000 new registered donors. 40% of Connecticut adults are have
registered as donors.
                                                                    FINANCIALS
For the second year in a row, this small non-profit organization struggled through a
difficult economy to obtain grants and other sources of funding to sustain and increase
outreach programs to broaden awareness of the need for organ and tissue donation.

The volunteer group of Donate Life Connecticut continues to add diversity to the
organization’s fundraising efforts through volunteer-driven projects such as Bowl for
Life, 5 Boro Bike Race, University of Bridgeport Basketball, ExerShar Personal Fitness,
memorial and honor gifts and countless others. New strategies were implemented to
expand revenue sources for the 2011 Annual Toast to Life fundraiser, now in its third
year, which included event sponsorship and diverse auction items. A new fundraiser
event added in 2011 was the Hartford ING Marathon. Not only did this organization
receive generous contribution as a result of the event, Donate Life Connecticut used
this opportunity to educate the running community about the importance of organ and
tissue donation.

                          The Mission:
The Connecticut Coalition for Organ and Tissue Donation, dba Donate
Life Connecticut (DLC), is dedicated to the purpose of public education
and awareness for all Connecticut residents concerning the lifesaving
benefits of organ and tissue donation and increasing the number of
registered donors.

                        The Membership:
Donate Life Connecticut is a state-wide collaborative organization
comprised of individuals devoted to increasing organ and tissue donation
awareness in the state. Members include donor families, transplant
recipients, healthcare professionals and supporters. The organization
addresses a spectrum of audiences ranging from high school and college
students, local businesses and churches, to state and local politicians.
Thanks to dramatic increases in public education, there are now over one
million registered donors in the state of Connecticut! Each person’s
decision to become a donor is critical…because each person has the power
to give the gift of life.
