UNT Hourly Employee Termination Form
Please use this form to terminate and remove hourly employees from payroll. Email completed form as an attachment to
HRRecords@untsystem.edu. HR Records will only accept and process terminations received on this form. If the form is not filled out completely, it
will be returned to the department for completion.
