1 Project Description
The purpose of this project is to implement Computerized Provider Order Entry (CPOE) across
UMMC. There are three deliverables associated with this project.

     A consolidated and standardized Order Sets to facilitate CPOE that are evidence based

     100% CPOE Compliance across the organization. Physicians will enter in real-time, all
      orders directly into FCIS thereby eliminating transcription and the use of third-person (i.e.
      Nurses, HUC etc.) to enter provider orders into patients’ electronic health records:
          Lab
          Imaging
          Consult
          Nutrition
          Medications
          Patient Care
          Etc,

     Decision Support Alerts – alerts to guide best practice, duplicate, drug interactions etc.
       Provide rules and alerts to guide care decisions
       Alerts for drug interactions
       On line help for alerts

2 Project Scope Statement
           The project is divided into three phases or sub-projects. First is the development of the
           consolidated Standardized Order Sets. The second phase is the implementation of 100%
           CPOE compliance across UMMC. The third phase is the implementation of decision
           support aids and alerts. One noted exception is the Physicians Informatics Committee
           may include the implementation of certain critical to patient safety decision support aids /
           alerts in phase one, the development of Order Sets.

           The project flow will start with developing order sets for patient population in the priority
           order shown in 2.2, then, CPOE will be initiated into that population. This roll between
           phase one and phase two will continue until all patient populations have been addressed.

           Phase three will commence after all efforts in phase one and two are complete.

           The following are in-scope for this project.

2.1       Order Set Development

          The UMMC Physicians Informatics Committee (PIC) is responsible for and has the
           authority to define and determine standardized order sets. Order Set are developed with

           the specific intent to support the implementation of CPOE across UMMC. Order Set
           development will be physician driven via the PIC

          The PIC will review existing order sets and define key new order sets for safety.

          Order Sets will focus within UMMC. The PIC may review activities at other Fairview
           facilities, coordinate and utilize other developed Order Sets as they feel appropriate.

          Order sets will utilize the following decision making priority in the selection and
           development and allowing for integration with eMAR
              o Patient Safety
                       Deal with the highly complex patient populations first
              o Comply with regulatory requirements
              o Improve provider work flows
              o Order Set Volume of Use – high volume sets before low volume sets.
              o Integrate with Nursing practices

          Order Sets to differentiate between themselves and protocols. Incorporate into protocols
           those standard orders associated with specific symptom-based patient populations.
               o Order sets triggered automatically for patient safety.

          Develop a single General Admission Order Set used across all of UMMC


2.2       Computerized Provider Order Entry (CPOE)

          100% compliance in utilizing CPOE across UMMC
          Physicians to enter orders directly in to FCIS including
              o Labs
              o RAD
              o Consult
              o Diets
              o Medications
              o Etc.
          Provide an order entry system to be used directly at the point of care
          Provide an order entry system that is directly incorporated into the patient’s electronic
           health record
          Roll out priority and schedule is:
           1. PEDS
           2. MED/SURG/CC/PAR
           3. OB
           4. Behavioral

          Some critical patient safety alerts will be developed and implemented during phases one
           and two, otherwise all other decision supports and alerts will be implemented after
           completion of phases one and two.
          Specific aids and alerts to be determined when phase three initiates.
          Alerts should flag documentation issues and contradictions.
          Provide proper output for coding and billing

2.4        Out of Scope – Exclusions Statement

          Cleaning up the backlog of maintenance request for order entry improvements and
           enhancements
          New order entry function related to FCIS (SCM) 4.5 upgrade
          Physician’s Non-Order Documentation
              o Structured Notes
              o Flowsheets
              o EMAR

3 Planning Considerations

          When project changes need to be made, this project will make decisions based on the
          following guidelines. The PIC represents the project sponsors on priority decisions.

4 Schedule

5 Costs
Increase in number of PCs and or moveable carts would increase the cost of the project. The
project plan will address additional technology requirements and resources to implement order
sets and provide CPOE access across UMMC

6 Change Control
The any proposed changes to this charter or scope must be approved by the project sponsor.

7 Status Report
Status reports will be email weekly to the project sponsors. Additional updates will be provided
to the executive steering committee by the program manager. The PIC will define status report
requirements for the committee.
