Short Answer FORM

							                 Children’s Hospital Colorado Dietetic Internship
              Affiliated with the University of Colorado and University of Colorado Hospital

All applicants to the Children’s Hospital Dietetic Internship must complete the following short-answer
questions and submit to the program with their cover letter and application fee. Absence of this
supplementary material will exclude applicant from consideration for this program. It is the
responsibility of the applicant to ensure that all materials are sent and received by the program by the
application deadline in the year for which they are applying.

Short-answer Question #1:

concentration areas offered through our program are Pediatrics and Adult Acute Care.
