Impact Concussion Test
Per Epping School District Policy all student athletes must take the Access Impact Baseline
Concussion Test. The test may be taken at home, or there will be selected school dates where the
labs will be open for computer use to complete the test. The test must be taken before your child
will be allowed to compete in any sport, and retaken if your child were to experience a
concussion during season.
Mr. Averill will be available after school to administer baseline test to those who need to take it.

Upon completion print documentation as proof of completion and provide Athletic Director with
copy. No student athlete will be allowed to compete in practice, or any school sponsored athletic
event without proof of completion of concussion test.
Drop off completed completion documentation at high school or middle school in main office, or
