                                 Proposal for ECG Machine
Aim of service change            The proposal is for the provision of an
                                 ECG Machine to provide ECG service
                                 within practice. The ECG Machine
                                 would allow more prompt diagnosis of
                                 conditions such as cardiac
                                 arrhythmias. Referrals will be
                                 reduced to secondary care providers
                                 namely Heanor Hospital and the
                                 Ambulatory Monitoring Department at
                                 Derby.
Expected improvements/benefits   The provision of the ECG Machine
                                 will enable patients to be identified
                                 who might otherwise not have been
                                 via usual practice routes. Health
                                 inequalities will be reduced. Patients
                                 will be seen initially in primary care
                                 reducing possible future secondary
                                 care episodes.
Service Delivery Model           The service will be locally based in
                                 general practice.

                                 adhered to
Potential Providers              The service is to be provided by a
                                 Health Care Assistant in General
                                 Practice.
Financial Input                  Cost of ECG Machine trolley based
                                 machine including trolley, installation
                                 and on site training:

                                 patients

                                 Improved accessibility will have
                                 positive impact on 18 week delivery
