SALARY: Grade 4 Points 13 – 19 (£15,444 - £17,802)

An exciting secondment/employment opportunity has arisen to join the Assistive
Technology Team to support the provision of a telecare service. The post is offered
on a 12 month fixed term basis.We are looking for a highly motivated dynamic
individual to join this busy team which will be based at Police HQ, Croesyceiliog.

You will develop and build on the existing skills of the current team who are
committed to providing a quality service. You will be required to work in partnership
with health, social care and housing colleagues to support service users to remain
safely in their own homes.

Duties for the post will include travelling throughout the Borough carrying out
assessments, simple installations and reviews of telecare equipment, as well as
other related tasks. You will provide advice and support to people in their own
homes. Full training will be given.This is an exciting opportunity for the right person
to work with a dedicated team of staff and be part of a developing service.A current
driving licence and use of transport is essential.

 Job Purpose

 To assist in the delivery of an effective assistive technology service to Torfaen
 residents. To assist in assessments, reviews and installations of assistive
 technology in the community.



Principal Accountabilities and Responsibilities

   1. To support the Assistive Technology Lead Assessor in undertaking non-
      complex telecare assessment visits and reviews, recording these
      appropriately and arranging installations.

   2. To review and prioritise referrals for Assistive Technology, working with Care
      Managers to consider risks, needs and appropriate Telecare packages.


   3. To complete simple installations of equipment in service users homes at the
      discretion of the Lead Assessor.


   4. Work in partnership with colleagues (especially Monitoring Centre staff) to
      ensure installations are completed on time and in accordance with the
      relevant agreements & standards.


   5. The Assistive Technology Assessor/Installer will also support the Lead
      Assessor in awareness raising and information events that promote assistive
      technology.
   6. To support the development of the Assistive Technology service, working with
       the Assistive Technology Lead Assessor



   7.To support awareness raising amongst stakeholders (including health, housing
      and social care professionals) of the potential use of Assistive technologies.

   8.To support operational arrangements with care managers to ensure that care
      plans reflect the assistive technology put in place and that reviews undertaken
      are also reflected in care plan documentation.

   9.To work with Lead Assessor to provide training to referring officers, users and
      carers on the operation of and uses of Telecare and Assistive technology.


   10. To carry out health and safety responsibilities in accordance with Torfaen
       County Borough Councils Policies and Procedures and the Health and Safety
       at Work act.

    review



General

 14. To be an effective team member and work within Corporate Standards and

     Chief Officer of Social Care and Housing.
