Subject: Annual Wireless Service Review (This is a requirement even if you obtained
your wireless service within the last six months.)
To All Wireless Service Users:
Per the Department of Administration Wireless Services Policy, an annual review must
be done by State Agencies of individual wireless service assignments to determine if
there is a continuing need and if it is cost justified. Over the past year we have noticed
several wireless devices that have not had any calls or data usage. Please take this
opportunity to evaluate if your device is needed.
To discontinue service on your wireless device, please follow the disconnection process
on the Wireless Services website and complete the Disposal Form. If you are having a
problem opening the links, do a Ctrl + Click.
To continue service on your wireless device, please review the DOA Policy, print and
complete the DOA Acknowledgement Policy, and print and complete the Approval
When reviewing the DOA Policy please note Section III A. Personal Use of Wireless
Services. Reimbursement for personal use should be sent to the Business Office after
your monthly review of the wireless services invoice. It is the user's responsibility to
review each monthly invoice and submit a Reimbursement Form for any personal calls
per the DOA policy.
The completed & signed DOA Acknowledgement Policy and Approval Form should be
returned to UW-Extension Purchasing; 432 N. Lake Street, Rm. 104; Madison, WI
53706-1498 by September 28, 2012. Non-compliance will result in the loss of your
