Client Quick Start Guide
for Adding an Earning/Deduction Code to Your
(Payentry.com and M3 Users)
Please be advised:
CBIZ will be responsible for making company level changes when notified
by the client to do so; clients are responsible for employee level changes.
1. Clients wishing to add a new Earning or Deduction code to their company will need to fill
out a New Code Setup Form. This form can be found at www.cbiz.com/payroll, click on
Resource Center, next to Payroll Resources change the drop down box to Payroll Forms
and click Go. Click Go next to New Code Setup Form.
2. Once you have the Setup Form opened you can either print it off, fill it out and fax it in or
fill it out on your PC and email it in to us at firstname.lastname@example.org.
3. In filling out the Form, CBIZ will need the following information from you on the Form.
First, tell us what code this is and how you want it abbreviated; i.e. – Child Support, CS; a
second Garnishment code, GARN2; Travel Reimbursement, TRVL, etc. Second, you will
need to indicate on the Form how this Earning/Deduction will be taxed. Third, indicate on
the Form if the Earning/Deduction should be included in a separate W2 box. Fourth,
advise us if an Earning code being set up should be included in your company’s 401K
calculation. Finally, fill in your company number, company name, sign and date.
