The OWNER / OCCUPIER of the building situated at and being xx.

WHEREAS it appears to the City of Bristol Council acting as the Local Authority for the
City of Bristol that the premises are in such a filthy and unwholesome condition as to be
prejudicial to health and, or are verminous.

NOTICE IS HEREBY GIVEN TO YOU that the said Local Authority requires you within the
space of xx from the date of the service of this notice to xx.

AND WE DO HEREBY GIVE YOU FURTHER NOTICE that if you fail to carry out the
above requirements within the said xx the Local Authority may themselves execute the
work hereby required and recover from you the expenses reasonably incurred in the
manner prescribed by the Public Health Act 1936.
