							                            Lindbergh Schweitzer Elementary PTA Craft Fair
                                         November 17, 2012
Terms and Conditions:
1. Only handcrafted items may be in the Fair. If you violate this policy, you may be asked to leave.
2. Space requests will be honored as available. Crafters who submit their applications by September 30, 2012 will be
   given first consideration. For these individuals, space numbers will be confirmed on October 1, 2012.
3. The Craft Show is a function of the PTA, not the school. Refer all inquiries to Hope Van de Wiele at hopevan@att.net.
4. The Craft Fair will take place from 9am to 3pm. Gates open at 7am for crafters to begin setting up. After
   unloading, crafters are expected to move their vehicles to the Crafters’ parking lot to ensure up front spaces for
   shoppers.
5. No pets please.
6. Refunds will be given up to one week before the event. After that date, your entry fee becomes a donation.
7. Craft personnel are solely responsible for their own possessions, wares and for the accounting of monies to any city,
   state and/or federal agency.

                               WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT
In consideration for receiving permission to participate in the 2012 LSEPTA CRAFT FAIR, the Vendor hereby releases,
waives, discharges and covenants not to sue The California State PTA -(LINDBERGH SCHWEITZER), its agents,
volunteers, or officers from any and all liability, claims, demands, actions and causes of action whatsoever arising from or
related to any loss, damage, or injury, including death, that may be sustained by the Vendor, or any of the property
belonging to the Vendor whether caused by the negligence of the Vendor/Releaser, or otherwise, while participating in
such activity, or while in, on or upon the premises where the activity is being conducted.

I am fully aware of the risks involved and hazards connected with the LSEPTA CRAFT FAIR and I hereby elect to
voluntarily participate in said activity with full knowledge that said activity may be hazardous to me and my property. I
voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including death, that may be
sustained by me or any loss or damage to property owned by me, as a result of being engaged in such activity, whether
caused by the negligence of Vendor/Releaser or otherwise.

I further hereby agree to indemnify and hold harmless the LSEPTA CRAFT FAIR, its agents, volunteers and officers from
any loss , liability, damage or costs, including court costs and attorney fees, that they may incur due to my participation in
said activity, whether caused by negligence of Vendor/Releaser or otherwise.

I understand that the LSEPTA CRAFT FAIR does not provide any insurance coverage for vendor participants for any
circumstances arising from their participation in this event or any activity associated with or facilitating that participation.
As such, I am aware that I should review my own insurance portfolio.
