By filling out this questionnaire it will help you pull together information to provide
the designer prior to receiving an estimate. Once received we’ll correspond until all
your needs are clearly defined and clarified.
Think of this step of the process as if you were building your own home. You wouldn’t
hand over a large sum of money to a builder and say “Build me a home but I don’t
know what kind or don’t have any preferences” would you? Of course not, and you
shouldn’t do it with a web designer either.
Your new website is about you and is an investment, but it is also an investment of
yourself, so take your time answering each of the questions below and provide as
much detailed information as possible. When you’re finished save this file as a
Microsoft Word doc file and email it to me at email@example.com as an email

Desired Pages for Your Site: What proposed site pages would you like for your site.
Think of this as if it were a table of contents with chapters and sub-chapters. For instance,
some common stand alone pages would be an “About” or “Contact” page but you may also
prefer a page which exhibits your “Portfolio” with sub-pages to your “Portraits” or

designer what is called content for your site. Content includes what you want to say about

covers information for such things as special instructions, disclaimers, terms and conditions,
shipping methods, FAQs, etc. Allow time to get it ready and ask for help if you need it.
Please understand that this part of the process is important and is your responsibility.
Progress cannot be made without your ALL your organized content submitted prior to the
start of the project.

Your Site Purpose: The focus and purpose of your website helps determine its
style. Some websites are text-driven, informal, or businesslike while others are
geared for visuals or the arts. Briefly describe your service, business, or special
interest and who your target audience is:
Please List Websites You Like: This helps the designer get a sense of your
style. List three websites on the internet that have a design you like and what
specific elements of the site appeal to you:
Color, Menus, and Blogs

would you prefer to leave that up to me?

Site Maintenance: For long-term maintenance a site usually evolves with
changes in your information or interests . Keeping a site updated will involve time
and money for this. What maintenance choice do you foresee after the project is
