Online ROO Form

USA Swimming’s ONLINE REPORT OF OCCURRENCE FORM

The Member Services Department of USA Swimming is pleased to announce
the completion of a new online version of the Report of Occurrence
(ROO) Form. Submitting the form online will be a more efficient way of
sending information to USA Swimming. The ROO is to be submitted any time
there is an accident or injury during such USA Swimming activities as
Sanctioned or Approved swim meets, swimming practices, contracted Swim-
a-Thons or approved social events. The online form does require that
certain sections, indicated by an asterisk, be completed before it can be
submitted. This new online report should be used effective immediately
when submitting a ROO. Reports previously submitted to USA Swimming
and the LSC Safety Coordinator do not need to be re-submitted.

Once the submitter has sent the form to USA Swimming AND received the
copy response of the report from USA Swimming, it should then be
forwarded via e-mail to Adam Hopkins, MS Safety Coordinator, who will
retain a copy for the MS LSC. If you have any questions, please contact
