Preconstruction Meeting Information for Districts on ARRA Projects
The Prime Contractors will be required to complete the online form FHWA-1589 by the 10th of each
month. The Contractor shall be responsible for reporting their company’s data, as well as every
Subcontractor’s data for every tier, including all Sub-Subcontractors.
The reporting requirements begin on the date of the signed contract. This may include reporting on
preparatory office work prior to the start of construction. The reporting requirements continue until the
Final Inspection Report (as defined in C&MS 109.12.A) is submitted for the project.
The Contractor will report the number of project direct-charged labor hours for their workforce and the
workforce of their Subcontractors active during the reporting month. This will include the workforce at
the project site as well as all office support (both in-house or outsources) performing services such as
but not limited to: CPM scheduling, QA/QC testing, fabrication design and review, SWPPP,
waste/borrow area site permits. Company Manager’s/Owners should also be reported when/if their
time is directly charged to the project. Material Suppliers need not be reported.
Due to some confusion with the language in the original ARRA proposal notes, there have been some
clarifications. All clarifications and current reporting requirements are being updated on the web site at:

To get started all prime contractors will need to follow the steps outlines below.
Contractor Steps to Submitting the 1589 Form Online
Step 1: fill out the “Account Request Form” link on the reporting page.
Step 2: Gather data for direct employees and subcontractors employees (number of employees,
hours, payroll dollars).
Step 3: Report employment data on FHWA-1589 form.
Step 4: Print copy of form, give to DCE with pay estimate.
Step 5: Click the “Submit” button to submit form online.

Please note: ODOT will be attempting to final these projects in 90 days in an effort to get any funds
remaining on the project re-encumbered. Please work with the Districts in making finalization of these
ARRA projects a priority.
