Position Available: Stark County Hospital and Community Liaison

Program:            Crisis Services

Supervisor:         Clinical Director

Hours of Work:      Full-time 40 hrs/week Monday - Friday

Responsibilities:   The primary purpose of this position is two fold: 1) Provide
                    leadership to the Stark County Bed Day Management Team
                    and oversee the utilization management of Heartland
                    Behavioral Health, in collaboration with the MHRSB of Stark
                    County and its contracted community-based behavioral
                    healthcare providers, for those clients on Stark County rolls,
                    and 2) Engage non-linked clients on the Crisis Stabilization
                    and Detoxification Units then provide on-going, community-
                    based case management services through linkage with an
                    identified provider. This will be done in accordance with
                    federal, state and local standards, guidelines, and
                    regulations that govern the agency such as Joint
                    Commission, ODMH, SCMHRSB, ODADAS, and AAS.

Qualifications:     Qualified candidates will have LSW or PC licensure in the
                    state of Ohio. Preferable candidates will have two (2) years
                    of experience in diagnostic assessment, crisis intervention
                    mental health, and experience working with individuals who
                    have a mental health / substance use / co-occurring
                    disorder(s). Must be skilled in case management (CPST),
                    assessment, diagnosis, ISP development, clinical
                    documentation, motivational interviewing, treatment of
                    mental and emotional disorders, data collection and
                    analysis, report writing, micro-macro collaborative care
                    planning, and an ability to establish and maintain
                    professional relationships in BH care arena. A valid driver’s
                    license and insurance and the ability to obtain CPR, First Aid
                    and NCI certification is also required.
