they kick off the weekend!

President’s Dinner and Gala

Deborah Tennyson. Dress in your “Denim and Diamonds” attire and turn heads as you enter for a fun
filled evening. Take your denim and “glam it up”! Be sure to stop by the photo booth with all of your

know the candidates and ask questions.

Open discussion on proposed resolutions and bylaws you need to attend.
Legislative Luncheon
Governmental Policy Group will update you on legislative issues. O-HY-PAC elections to take place.
Table Clinic Hall
Come listen to our students with their presentations on today’s issues. Be enlightened by our future
Dental Hygienists; Judging will be done prior to viewing.
Exhibit Hall
Exhibitors will be spread throughout the hallways and student and component marketplace will be in
the Flint room. Come, shop and see the new products.
Professional Issues Forum
Attend the Professional Issues forum facilitated by our ODHA Leadership. Do YOU know what is
happening in your profession?
Student Reception
Students will be competing in the trivia game “Are You Smarter Than Esther Wilkins?”

                           ODHA Service Award
Continued service to one’s professional organization is vital to the success of that organization.
Members often serve the organization unselfishly without receiving formal recognition or thanks for
their efforts. The ODHA Service Award is designed to recognize a member who has given unselfishly
to the STATE Dental Hygienist’ Association for five or more years. Any ODHA member may be
nominated if the following criteria are met:

1. Five years or more of ODHA membership to include:
     a) Service as an ODHA Council Chair, Trustee, and/or Committee Member.
     b) Attendance at the 2012 ODHA Annual Session.
     c) Attendance at several past ODHA support workshops and/or continuing education programs.
2. Volunteer efforts for ODHA through one or more of the following ways:
     a) Special committee work.
     b) Planning special ODHA programs.
     c) Community oriented efforts representing ODHA.
     d) Public relations for ODHA.
     e) Legislative activities.
3. Any special assignment accepted by the member which has been sanctioned by ODHA but
      not specified in the stated criteria.
It is necessary to include a written testimonial of why you believe this member is deserving of this
award, using the criteria listed above.



             ODHA Outstanding Effort Award
This award is designed to honor a selected member or ODHA Committee who has best exemplified
outstanding service to the STATE Dental Hygienists’ Association during the 2011-2012 fiscal year.
An individual or Committee may be nominated for this award if the following criteria are met:
          1. Membership on the ODHA Executive Board, Board of Trustees, ODHA Council, and/or
              ODHA Committee.
          2. Attendance at the 2012 ODHA Annual Session.
          3. Attendance at no less than three (3) Board of Trustees meetings during 2011-2012.
          4. Participation in at least one (1) ODHA sponsored workshop and/or continuing education
              program during 2011-12.
          5. Volunteers time and effort “beyond the call of duty” to further advance the profession of
              Dental Hygiene in Ohio.

It is necessary to include a written testimonial of why you feel this member or committee is deserving
of this award, using the criteria listed above.

Deadline to submit award applications are 10/10/12.
