									Robert Zed Young Health Leader Award


The Award
The Robert Zed Young Health Leader Award is presented to a Canadian healthcare leader who
has demonstrated leadership in the effectiveness and sustainability of Canada’s health system.
The winner will receive an etched glass trophy, paid travel and accommodations to attend the
College’s National Awards Gala and a complimentary registration to the National Health
Leadership Conference.

Eligibility
Nominations are invited for leaders, age 40 or younger and must satisfy the following criteria:
     be a member of the College;
     has, in the last 5-10 years, shown leadership growth and advancement in their
         healthcare career;
     demonstrates potential for senior healthcare leadership positions in the future, in any
         area of health services, including public health, health prevention, health promotion,
         acute care, long term care, rehabilitation, mental health and drug dependency services,
         national, provincial or local health organizations, health charities, private sector health
         organizations or any related area; and
     has demonstrated outstanding vision and leadership through a project, program,
         publication, community involvement, philanthropy, or other activity that has resulted in
         measurable improvements within their organization and/or within the health system.

Nominations

Nominations must be made by an individual member or a chapter. Should a chapter’s
nomination be successful, the chapter will receive a $500 education grant. New this year, we
are asking nominators to submit their nomination in two sections. Section A, which is to be
submitted by November 16, 2012, is a 250 word summary. The summary should touch on the
eligibility criteria and illustrate why you feel that the nominee should win this award. The
summaries will be reviewed by the selection committee and a short list of top contenders will
be formed. Those on the shortlist will be asked to complete Section B of the nomination, found
below. Section B is due on February 1, 2013.

Section B of the nomination package should include:
    the completed nomination form;
    the nomination summary;
    a letter of support from the nominee’s chief executive officer;
    a copy of the nominee’s résumé; and
    using the report template, describe how the nominee meets the eligibility criteria

Please include a synopsis of the nominee’s achievements, as they align to the award criteria and
description. This information will be reviewed by the selection committee in order to create a
shortlist of top contenders. It will also be published in the College’s Leading Practices booklet.

   Please provide a history of the nominee’s involvement with the College.

4. Discuss the nominee’s professional development plan, using the five key capabilities in
   the LEADS in a Caring Environment capabilities framework, as headers: (15 POINTS)
    Lead Self
    Engage Others
    Achieve Results
    Develop Coalitions
    Systems Transformation

5. Discuss notable accomplishments achieved thus far in the nominee’s career.
Please note the impact that these achievements have had on:
        improvement in patient outcomes;(10 POINTS)
        quality measures; (10 POINTS)
        financial cost reductions, or securing additional funding. (10 POINTS)

6. Conclusion
How to submit your nomination:

The nomination should include all of the items listed below. The nomination is to be submitted to
cmacbride@cchl-ccls.ca as one PDF file. The letter of support and any additional information should
be included in the PDF file as well. Electronic signatures are acceptable.

            the completed nomination template;
            a letter of support from the nominee’s CEO; and
            any additional information you feel may be important for the selection committee’s

             support, etc. as an appendix.).
