          In order for your application to be considered, all of the following must be sent to Headquarters
                                           Postmarked by April 10, 2006.
  Late materials will not be considered, and extensions will not be granted. To assist you, fill in the date that each
                           requirement was sent or requested from the appropriate sources.

    (Educational Grant Applicants only)

     A letter acknowledging the receipt of your application materials will be sent to you after your application
materials are received in Headquarters. If you do not receive a letter within two weeks of submitting your application,
please contact the Foundation Scholarship Committee immediately.

     The award winners will be announced in August 2006.
