							                                        We need valid
                                           proofs of
                                           residence
                                        before you can
                                            register.

I. You must present (1) of the following proofs that reflect your
physical residence.
   The physical address must be visible.
         1. Property tax records or deeds / Tax receipt record or deed
         2. Apartment or house lease / Rent receipt or lease
         3. Mortgage statement

II. You must also present (1) of the following:
    The physical address must be visible.
          1. Utility bill: water, gas, land line phone, electric, cable bill,
             garbage bill or deposit receipts
          2. Income tax record from IRS to parent or guardian. Copy of
             check / correspondence from IRS with the physical address
             visible.
          3. Bank records: Checking, saving, loan statements, investment
             certificates
          4. Employment Record: Company check from employer with
             your physical address on it.
          5. Two Credit Card Account statements. These (2) statements
             count as one proof.
          6. Correspondence from the Social Security Office, Department of
             Human Resources, (DHR), Food Stamp Office or Temporary
             Assistance to Needy Families (TANF)

   III.   Your proof of residence for the Forest Hill Elementary School
          district will be verified and copied prior to completing any
          paperwork. Once given an admit slip you will be asked to
          proceed to the designated area to register your child.
