                                 REFERRAL PROCEDURE

INTRODUCTION

The referral procedure package has been developed in order to assist management in
the process of referral to the Occupational Health Department.

Whilst the procedure package is aimed to management referrals, it must be
remembered that staff can refer themselves directly to the Occupational Health
Department for help and/or advice.

It must also be emphasized that the role of the Occupational Health Department is to
provide advice to the Trust. The responsibility of deciding what action to take as a
result of this advice rests with management.

Managers are encouraged to telephone Occupational Health on the number
below prior to referral, if they feel clarification or support is required.

REFERRAL GUIDELINES

The reasons for referral are many and varied, consequently this section has been
drawn up to guide managers as to why an Occupational Health referral is required and
what are they requesting of the Occupational Health Physician/Nurse.

When making an official written referral, management must consider the following:

*         Why is a referral being made?

          These are only examples, other circumstances may arise:

          (i)      Sickness absence - concern with regards the length or frequency?
          (ii)     Deterioration in work performance - is this health related?
          (iii)    Is health having an effect on work or vice versa?
          (iv)     Fitness to continue in post?
          (v)      Return to work - full duties, gradual return, redeployment or ill health
                   retirement?

*         What advice is being asked of the Physician/Nurse? For example:

          (i)      Is there a health problem management should be aware of which may
                   affect work or be exacerbated by work?
          (ii)     Can the employee return to work following absence?
          (iii)    Should this employee be redeployed, have their hours reduced or
                   re-trained?
          (iv)     Is ill health retirement necessary?

*         What help do you require from the Physician/Nurse? For example:

          (i)      Are there any limitations on the tasks or work that this employee
                   performs? Is so, what are they?
          (ii)     Are they fit to continue in employment?
          (iii)    Are the limitations short or long-term?

*         Can I provide the Physician/Advisor with any other information to assist
          in the assessment of the staff’s health? For example:

          (i)      Past history
          (ii)     Record of sickness absence
          (iii)    Social circumstances
          (iv)     Job description
          (v)      Detailed letter

Management must also consider:

          (a)      Is this really a health related issue?
          (b)      Is this a management/personnel issue?
          (c)      Is it a combination of both?

Whilst it is difficult to separate the two, at times consideration is essential to avoid staff
being referred to Occupational Health with management issues.

Written consent for further information from their GP, Specialist or Consultant etc will
be obtained by the Occupational Health Department at the time of consultation with
member of staff if required.

REFERRAL PROCESS

The Manager

1.        Read notes on the referral guidelines.

2.        Communicate with the employee as to why a referral is being made and
          provide them with the ‘Staff information sheet on referral to Occupational
          Health’.

3.        Complete the referral form ensuring as much information to enable an
          assessment has been recorded including ‘check list’ section.

Occupational Health

1.        Upon receipt of a referral an appointment to see the Occupational Health
          Physician/Advisor will be sent directly to the member of staff.

2.        Following the appointment with the Occupational Health Physician/Nurse, the
          manager will receive a written response. Each case will have its own individual
          circumstances.

3.        Occupational Health will arrange any follow-up and/or gain more information
          as necessary.

Standards

1.        Receipt of referral will be recorded, date stamped and passed to OH Advisor
          for assessment. This should occur within 24-48 hours of Occupational Health
          working days (excluding weekends and Bank Holidays).

2.        Following assessment of the Referral form by OH Advisor one of two actions
          will occur:

          2.1      An appointment to see OH Advisor– within 7 working days.

          2.2      An appointment to see OH Physician – within 14 working days.

3.        H.R, referring manager and member of staff will all be notified of receipt of
          referral and appointment within 24-48 occupational health working days.

4.        Outcome of appointment – A letter will be sent to referring manager and copy
          to H.R. stating :

          4.1      Identify current level of fitness for work.

          4.2      State likely date of return to work (if not possible to do so – state why
                   not possible).

          4.3      Specify limitations for return to work.

          4.4      Identify existence of any underlying medical conditions, which may
                   contribute to unsatisfactory attendance, performance, safety.

          4.5      State if Disability Discrimination Act applies or not.

          4.6      Give advice reference rehabilitation.

          4.7      If to be reviewed include next appointment.

Is the member of staff currently involved any grievance/discipline/dignity at work
issues? If relevant, please provide details.




Required outcome of referral and specific questions to be asked e.g. prognosis
for return to work, timescale, appropriateness of rehabilitation programme,
adjustments to role, implications under the DDA, long term prognosis.

                           STAFF INFORMATION SHEET


Following a discussion with your manager, you will now be aware that they wish for you
to see a member of the Occupational Health Team.

So what happens now?

You will receive a letter with an appointment to see the Occupational Health
Physician/Nurse.

At this appointment, the Occupational Health Physician/Advisor will discuss the issues
raised by the referral and make an assessment on your health in relation to your work.

You may be asked at the time of the appointment to give permission for the
Occupational Health Physician/Advisor to contact your general practitioner, consultant
or specialist etc, who have been treating you.

The provision of this information will assist the Occupational Health Physician/Advisor
in their assessment of the situation, and to enable him/her to advise yourself and
management on the issues of your health and work.
