					                                           Observed Cultural Intelligence Questionnaire

To the student in the Cross Cultural Differences in Organizational Behavior Class:

This questionnaire helps you obtain information about your cultural intelligence (CQ) from someone who knows you well. Such
information will enable you to gain more accurate information about your CQ. This is the same questionnaire you completed in the
self-assessment part of your cultural intelligence.

Give this questionnaire to someone you trust and whose opinion you respect. Ask this person to complete the questionnaire with
you in mind. Tell this person that you will use it to develop your cultural intelligence competence and that it is part of your
coursework in the class and that it is very important that they answer it as honestly as possible. Assure them that only you will be
able to see their response

Fill in your name in the blanks provided on the top of page 2 of this document before giving it to the person you choose.

Only give page 2 to the person who will rate you.

After the person you choose completes this questionnaire, you can score their responses and compare your own assessment of
your cultural intelligence skills that person’s assessment of your cultural intelligence.
Thank you for agreeing to complete this survey for ___________________________ as part of an exercise for cross cultural competence
leadership course. Your responses to this survey will be very useful to _______________________________. Please note that this survey
is completely confidential and anonymous. Only the person who gave it to you will be able to see your responses. Your responses will not
affect the person’s course grades in any way.

Any items you may object to can be left blank. After completing this survey, please return it to the person you are rating.

Please describe _______________________________ as s/he is generally now, not as you wish them to be, or what they wish to be in the
future. Describe ____________________ as you honestly see him/her, in relation to other people similar to him/her with respect to sex, and
age.

Below are phrases describing people's behaviors. Please read each statement carefully, and then select the response that BEST describes
______________________ by marking the appropriate number on the scale beside each question. If you are in the same class as the
person you are describing can use that person’s behavior in the BafaBafa exercise or other contexts you may know that person. If you don’t
have any information about the person leave it blank. Use the following format:
