Will, all of you, who have revised the nutritional risk screen, email me a sample?
Here is a copy of Connecticut's Nutritional Risk assessment form. We have not revised
it this year because we are waiting for official word from our national government, which
I understand may take an act of Congress. Making changes is very costly because
nutrition projects must modify their computer systems to accommodate them.
We do have a problem with duplication. Our assessment form has four purposes: (1)
to satisfy NAPIS or POMP requirements, (2) to identify problems participants might
have so that we can help them, (3) to obtain a nutritional risk score that is somewhat
responsive to programming so that we can measure our performance, and (3) to rank
participants on a waiting list.
If our national government would allow us to discard or modify the NAPIS questions, we
could develop a set of questions that would satisfy needs 2 and 3, avoid duplication,
and save a lot of people a lot of time.
