Instructions


Complete an evaluation form for each vendor that has submitted a proposal. If the proposal is non-responsive,
note the reason (s) / specifics in the notes / additional comments area. Attach additional pages if needed.


The evaluation criteria are the same criteria listed in the RFP. For criteria 1 through 5, rate the proposal on a
scale of 0 to 10, with 0 being the lowest score and 10 being the highest score. To calculate the weighted score,
multiply the score by the weight. For criterion 6 (Cost), divide the lowest proposal price by the proposal price
being scored, then multiply the result by the weight. Total the weighted scores at the bottom.


Please provide the comments to substantiate your scores. Attach additional pages if necessary.


When all the reviewers have created their scores electronically, enter totals from weighted score under
their reviewer number in the committee rankings worksheet. (NOTE: be sure to enter number of reviewers in yellow box)
