                               Request for Information (RFI)

Statement of Need

The Florida Department of Management Services’ Division of State Purchasing is issuing a Request for Information (RFI)
to Florida’s vendor community to obtain industry information about the management consulting, benefit consulting,
and actuarial services categories.

Background

Florida's purchasing power allows the Division of State Purchasing to deliver the best value in goods and services for
state agencies and local governments. The Division strives to develop and implement sound procurement practices
throughout the state and is dedicated to building strong relationships with all state agencies, local governments, and
vendors. The Division promotes fair and open competition in the state's procurement process and provides professional
leadership and guidance in understanding and using the best resources available.

RFI Information and Due Date

Responses to this RFI will be collected using this Microsoft Excel workbook. Please click on each tab of the MS Excel
workbook, starting with "Company Information" and respond to the questions asked on each worksheet. For "Yes/No"
questions, please click on the drop-down arrow to select your response. In the text box beneath each question, please
type your response.

Provided with the MS Excel version of the RFI are two supplemental documents that you will use to complete your
response. Please review Attachment A - Category Descriptions before completing the "Contract Categories" worksheet
and review Attachment B - Job Title Descriptions before completing the "Job Titles & Pricing Info" worksheet. These
attachments contain information that will be necessary to provide a complete response to the RFI.

When you have answered all of the RFI questions, please save the file to your computer and send a copy to Trey Collins
at trey.collins@dms.myflorida.com.

We appreciate your completing this RFI by no later than Wednesday, March 7, 2012, and estimate it will take
approximately 25 minutes to answer the questions. You may forward this RFI to someone else within your company to
complete if appropriate.
